子宫内膜息肉合并不孕症患者宫腔镜术后妊娠结局的影响因素

Influencing Factors of Pregnancy Outcome in Infertile Patients with Endometrial Polyps After Hysteroscopic Surgery

目的 分析子宫内膜息肉合并不孕症患者经宫腔镜手术治疗后的妊娠结局及影响因素。方法 选取洛阳市妇幼保健院2019年9月至2021年9月收治的经宫腔镜手术治疗子宫内膜息肉后自然妊娠的40例孕妇纳入妊娠组。选取医院同期收治的经宫腔镜手术治疗子宫内膜息肉后未妊娠的32例孕妇纳入未妊娠组。收集两组患者的临床资料:年龄、体质量指数、不孕类型、不孕年限、合并基础疾病、术后子宫内膜厚度、息肉大小、术后月经量等;采用logistic回归分析影响子宫内膜息肉合并不孕症患者宫腔镜手术治疗后妊娠结局的因素。结果 两组患者年龄、术后子宫内膜厚度、合并糖尿病以及息肉大小比较,差异有统计学意义(P<0.05),其余一般资料比较,差异无统计学意义(P>0.05)。logistic回归分析结果显示年龄、术后子宫内膜厚度、合并糖尿病以及息肉大小是影响患者术后妊娠结局的因素。结论 子宫内膜息肉合并不孕症患者经宫腔镜治疗后其妊娠结局受年龄、术后子宫内膜厚度、合并糖尿病以及息肉大小方面影响。

Objective To analyze the pregnancy outcome and influencing factors of infertility patients with endometrial polyps after hysteroscopic surgery.Methods Forty pregnant women who were admitted to Luoyang Maternal and Child Health Care Hospital from September 2019 to September 2021 for spontaneous pregnancy after hysteroscopic surgery for endometrial polyps were selected to be included in the pregnancy group. Thirty two pregnant women who were not pregnant after hysteroscopic surgery for endometrial polyps were enrolled in the non pregnancy group. The clinical data of the two groups were collected, including age, body mass index, type of infertility, years of infertility, combined basic diseases, endometrial thickness, polyp size, and menstrual volume after surgery. Logistic regression analysis was used to analyze the factors influencing the pregnancy outcome of infertility patients with endometrial polyps after hysteroscopy.Results There was statistically significant difference between the two groups in terms of age, postoperative endometrial thickness, combined diabetes and polyp size(P<0.05), while there were no statistically significant differences in other general information(P>0.05). Logistic regression analysis showed that age, endometrial thickness, diabetes and polyp size were the factors influencing the pregnancy outcome.Conclusion The pregnancy outcome of infertile patients with endometrial polyps after hysteroscopic treatment is affected by age, postoperative endometrial thickness, diabetes and polyp size.
