家庭延伸护理方式对慢性心力衰竭伴心律失常患者自我效能及自护能力的影响研究

Study on the Effect of Extended Home Care Approach on Self-efficacy and Self-care Ability of Patients with Chronic Heart Failure with Arrhythmias

目的:探讨家庭延伸护理方式对慢性心力衰竭伴心律失常患者自我效能及自护能力的影响。方法:选取2018年9月―2020年9月河南省中医院收治的106例慢性心力衰竭伴心律失常患者作为研究对象,按随机数表法分为对照组和观察组,每组各53例。对照组实施常规护理,观察组实施家庭延伸护理。比较两组患者医嘱依从性、负性情绪、自我效能及自护能力情况。结果:观察组医嘱依从性高于对照组,差异有统计学意义(Z=2.643,P<0.05)。干预后,观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于对照组,差异有统计学意义(t=9.041、8.323,P<0.05)。干预后,观察组自我效能感量表(GSES)评分、自我护理能力测定量表(ESCA)中各维度评分及总分均高于对照组,差异有统计学意义(t=8.313、5.547、8.668、7.752、8.039、9.685,P<0.05)。结论:家庭延伸护理能够提高慢性心力衰竭伴心律失常患者遵医行为,缓解患者不良情绪,改善其自我效能,增强其自护能力。

Objective:To explore the effect of extended home care modality on self-efficacy and self-care in patients with chronic heart failure with arrhythmias.Methods:106 patients with chronic heart failure with arrhythmias admitted to the hospital from September 2018 to September 2020 were selected as study subjects and divided into control and observation groups according to the random number table method, with 53 cases in each group. Conventional care was implemented in the control group and extended home care was implemented in the observation group. The patients’ compliance with medical advice, negative emotions,self-efficacy and self-care ability were compared between the two groups.Results:The compliance with medical advice was higher in the observation group than in the control group, and the difference was statistically significant(Z=2.643, P<0.05). After the intervention, SAS and SDS scores were lower in the observation group than in the control group, with statistically significant differences(t=9.041, 8.323, P<0.05). After the intervention, the GSES score, the score of each dimension in ESCA and the total score were higher in the observation group than in the control group, and the difference was statistically significant(t=8.313, 5.547, 8.668,7.752, 8.039, 9.685, P<0.05).Conclusion:Extended home care can improve the compliance behavior of patients with chronic heart failure with arrhythmias, relieve patients’ adverse emotions, improve their self-efficacy, and enhance their self-care ability.
