止痛如神汤内服联合康复新液熏洗对重度混合痔术后患者水肿、创面愈合及炎症指标的影响

Effects of Oral Administration of Zhitong Rushen Decoction Combined with Kangfuxin Liquid Fumigation and Washing on Edema, Wound Healing and Inflammatory Indexes in Patients with Severe Mixed Hemorrhoids after Operation

目的 分析止痛如神汤内服联合康复新液熏洗对重度混合痔术后患者水肿、创面愈合及炎症指标的影响。方法 方便选取2021年1—10月于南平市建阳第一医院进行手术治疗的重度混合痔患者75例为研究对象,采用随机数字表法将其分为对照组(n=37)和观察组(n=38),对照组采用康复新液熏洗治疗,观察组在对照组的基础上增加止痛如神汤内服,对比两组临床疗效。结果 观察组创面疼痛缓解时间(3.43±1.51)d、肉芽组织与创面相平时间(12.87±1.34)d、创面愈合时间(18.85±2.24)d均较对照组明显缩短,差异有统计学意义(t=6.360、3.249、4.739,P<0.05);观察组术后3、7、21 d的肛门水肿评分为(2.21±0.27)分、(1.52±0.33)分、(0.50±0.14)分,均低于对照组,差异有统计学意义(t=4.238、3.527、9.651,P<0.05);术后21 d,相较对照组,观察组C反应蛋白(21.11±3.25)mg/L、白细胞介素-6(12.75±1.69)pg/mL、肿瘤坏死因子-α水平(18.75±2.15)pg/mL明显降低,差异有统计学意义(t=6.318、9.104、9.522,P<0.05)。结论 重度混合痔患者经手术治疗后接受止痛如神汤内服联合康复新液熏洗治疗,可有效改善肛门水肿情况,缩短康复时间,消除炎症反应,效果显著。

Objective To analyze the effect of Zhitong Rushen decoction combined with Kangfuxin liquid fumigation and washing on edema, wound healing and inflammatory indexes in patients with severe mixed hemorrhoids after operation. Methods A total of 75 patients with severe mixed hemorrhoids who underwent surgical treatment in the Jianyang First Hospital in Nanping city from January to October 2021 were conveniently selected as the research object.They were divided into control group(n=37) and observation group(n=38) by random number table method. The control group was treated with Kangfuxin liquid fumigation and washing, and the observation group was added with Zhitong Rushen Decoction orally on the basis of the control group, and the clinical efficacy was compared between the two groups. Results The pain relief time of the observation group was(3.43±1.51) d, the time between the granulation tissue and the wound surface was(12.87±1.34) d, and the wound healing time(18.85±2.24) d was significantly shorter than that of the control group, the difference was statistically significant(t=6.360, 3.249, 4.739, P<0.05). The scores of anal edema(2.21±0.27) points,(1.52±0.33) points,(0.50±0.14) points in the observation group at postoperative 3 d, 7 d and 21 d were lower than those in the control group, the difference was statistically significant(t=4.238, 3.527, 9.651,P<0.05). On the 21st day after operation, compared with the control group, the levels of C-reactive protein(21.11±3.25) mg/L, interleukin-6(12.75±1.69) pg/mL and tumor necrosis factor-α(18.75±2.15) pg/mL in the observation group were significantly decreased, the difference was statistically significant(t=6.318, 9.104, 9.522, P<0.05).Conclusion After surgical treatment of severe mixed hemorrhoids, the oral administration of Zhitong Rushen decoction combined with Fufuxin liquid fumigation and washing can effectively improve the anal edema, shorten the recovery time, and eliminate the inflammatory reaction, and the effect is remarkable.
