口腔黏膜癌前病变合并白假丝酵母感染患者 HBD-2及LL-37水平及基因表达 被引量:1

Expressions of HBD-2 and LL-37 in oral mucosal precancerous lesion patients complicated with Candida albicans infection

摘要 目的 探讨白假丝酵母感染对口腔黏膜癌前病变患者唾液中人类β防御素-2(HBD-2)、抗菌肽LL-37水平和基因表达的影响。方法 选取2017年10月-2021年1月宁波市医疗中心李惠利医院收治的口腔黏膜癌前病变合并白假丝酵母感染患者66例为感染组,另选取同期收治无感染口腔黏膜癌前病变患者132例为非感染组,酶联免疫吸附法检测唾液HBD-2和LL-37水平,逆转录聚合酶链式反应(RT-PCR)检测唾液中HBD-2和LL-37基因相对表达量,使用受试者工作特征(ROC)曲线分析HBD-2和LL-37诊断口腔黏膜癌前病变合并白假丝酵母感染的临床价值。结果 感染组口腔黏膜癌前病变患者上皮异常增生占比高于非感染组(P<0.05);感染组患者唾液HBD-2和LL-37水平高于非感染组,基因相对表达量高于非感染组(P<0.05);HBD-2和LL-37水平预测口腔黏膜癌前病变合并白假丝酵母感染的曲线下面积(AUC)分别为0.768和0.744,基因相对表达量的AUC分别为0.723和0.734。结论 口腔黏膜癌前病变合并白假丝酵母感染患者唾液中HBD-2、LL-37水平和基因相对表达量均升高,并且具有良好的预测价值。 OBJECTIVE To explore the influence of Candida albicans infection on expression levels of human β-defensin-2(HBD-2) and antimicrobial peptide LL-37 genes in saliva of patients with oral mucosal precancerous lesions. METHODS Totally 66 oral mucosal precancerous lesion patients complicated with C. albicans infection who were treated in Lihuili Hospital, Ningbo Medical Center from Oct 2017 to Jan 2021 were assigned as the infection group, meanwhile, 132 patients with oral mucosal precancerous lesions who did not have infection were chosen as the non-infection group. The levels of HBD-2 and LL-37 in saliva specimens were detected by enzyme-linked immunosorbent assay, the relative expression levels of HBD-2 and LL-37 genes in saliva specimens were detected by means of reverse transcription polymerase chain reaction(RT-PCR), and the clinical values of HBD-2 and LL-3 in diagnosis of C. albicans infection in the oral mucosal precancerous lesions were analyzed by using receiver operating characteristic(ROC) curves. RESULTS The proportion of the oral mucosal precancerous lesion patients who had epithelial dysplasia was higher in the infection group than in the non-infection group(P<0.05). The levels of saliva HBD-2 and LL-37 of the infection group were higher than those of the non-infection group, and the relative expression levels of the genes of the infection group were higher than those of the non-infection group(P<0.05). The areas under curves(AUCs) of HBD-2 and LL-37 were respectively 0.768 and 0.744 in prediction of C. albicans infection in the patients with oral mucosal precancerous lesions, and the AUCs of the relative expression levels of the genes were 0.723 and 0.734, respectively. CONCLUSION The oral mucosal precancerous lesion patients complicated with C. albicans infection show the rise of levels of HBD-2, LL-37, and relative expression levels of the genes in saliva, which have high value in prediction of the infection.
