摘要
目的 分析儿童腺病毒与甲型、乙型流感病毒感染临床特征及实验室指标,为儿童腺病毒及甲、乙型流感病毒感染的识别、诊断及治疗提供参考。方法 选取2019年1月-6月医院收治入院的69例甲型流感患儿(甲流组)、62例乙型流感患儿(乙流组)及50例腺病毒感染患儿(腺病毒组)为研究对象,回顾性分析各组患儿临床特征及实验室检查结果的差异。结果 三组患儿年龄比较,差异有统计学意义(P<0.05),腺病毒组患儿主要分布在29天~1岁和2~7岁,甲流组和乙流组患儿主要分布在2~7岁;三组患儿性别比较,差异无统计学意义;三组患儿均以39.0℃以上高热为主;三组患儿气促呼吸困难占比差异均有统计学意义(P<0.05),其他症状差异无统计学意义;腺病毒组白细胞计数(WBC)、乳酸脱氢酶(LDH)和C-反应蛋白(CRP)水平均高于甲流组和乙流组(P<0.05),乙流组红细胞计数(RBC)、血红蛋白(HGB)和单核细胞百分比(MONO%)水平均高于甲流组和腺病毒组(P<0.05)。结论 腺病毒感染患儿的热峰、血象指标、LDH水平高于甲流组和乙流组,联合检测血常规、LDH可加强对这两种病毒感染患儿的早期识别,为疾病的诊断及鉴别诊断提供一定的依据。
OBJECTIVE To investigate the clinical characteristics of children with adenovirus infection, influenza A virus infection, influenza B virus infection and the laboratory test indexes so as to provide guidance for differential diagnosis and treatment. METHODS A total of 69 children with influenza who were treated in the hospital from Jan 2019 to Jun 2019 were assigned as the influenza A group, 62 children with influenza B were chosen as the influenza B group, and 50 children with adenovirus infection were assigned as the adenovirus group. The clinical characteristics and laboratory test indexes were observed and compared among the three groups of children. RESULTS There was significant difference in the age among the three groups of children(P<0.05), the age of the children in the adenovirus group ranged between 29 days and 1 year old as well as 2 years old and 7 years old, the age of the children in the influenza A group and the influenza B group mainly ranged between 2 years old and 7 years old. There was no significant difference in the gender among the three groups of children. The hyperpyrexia children with body temperature more than 39.0℃ were dominant among the three groups of children;there was significant difference in the proportion of children with respiratory dyspnea among the three groups(P<0.05), while there were no significant differences in the proportions of children with other symptoms. The levels of white blood cell(WBC), lactic dehydrogenase(LDH) and C-reactive protein(CRP) of the adenovirus group were higher than those of the influenza A group and the influenza B group(P<0.05);the levels of red blood cell(RBC), hemoglobin(HGB) and percentage of monocyte(MONO%) of the influenza B group were higher than those of the influenza A group and the adenovirus group(P<0.05). CONCLUSION The heat peak, blood indexes and LDH levels of the children with adenovirus infection are higher than those of the children with influenza A virus infection and the children with influenza B virus infection. The joint detection of blood routine indexes and LDH may facilitate the identification of the two types of infections in early stage and provide guidance for the differential diagnosis.
