Fas/Fasl基因多态性与急性心肌梗死患者术后肺部感染的关联 被引量:1

Correlation between Fas/Fasl gene polymorphisms and postoperative pulmonary infection in patients with acute myocardial infarction

摘要 目的探究Fas/Fasl基因多态性与急性心肌梗死患者术后肺部感染的关联。方法选取2017年3月-2020年12月首都医科大学附属北京世纪坛医院收治的急性心肌梗死手术患者155例,按照术后是否并发肺部感染将患者分为感染组21例和未感染组134例,对两组Fas基因rs1571013、rs1800682位点和Fasl基因rs6700734、rs763110位点基因多态性进行检测,采用Hardy-weinberg遗传平衡检验分析上述基因位点分布情况,对比两组基因分布差异,并评价Fas/Fasl基因多态性与术后肺部感染发生的关系。结果两组患者Fas/Fasl基因型符合遗传平衡定律,感染组rs1571013位点上A/A基因型、等位基因A和rs1800682位点上A/G基因型、等位基因G的分布频率显著高于未感染组(P<0.05);两组Fasl基因rs6700734位点和rs763110位点基因型及等位基因的分布无统计学差异;rs1571013位点A/A基因型和A等位基因发生术后肺部感染的风险高于G/G基因型和G等位基因(P<0.05);rs1800682位点G/G基因型、A/G基因型和G等位基因发生术后肺部感染的风险高于A/A基因型和A等位基因(P<0.05)。结论Fas基因rs1571013、rs1800682位点与急性心肌梗死患者术后肺部感染的发生有关,监测基因型和相关位点有助于临床预测术后肺部感染。 OBJECTIVE To explore the correlation between Fas/Fasl gene polymorphisms and postoperative pulmonary infection in patients with acute myocardial infarction.METHODS A total of 155 patients with acute myocardial infarction who underwent surgery in Beijing Shijitan Hospital Affiliated to Capital Medical University from Mar 2017 to Dec 2020 were enrolled in the study and divided into the infection group with 21 cases and the no infection group with 134 cases according to the status of postoperative pulmonary infection.The gene polymorphisms at rs1571013 and rs1800682 loci of Fas gene as well as rs6700734 and rs763110 loci of Fasl gene were detected for the two groups of patients,the loci of the above genes were analyzed by means of Hardy-weinberg genetic equilibrium test,the distribution of genes was compared between the two groups,and the relationship between the Fas/Fasl gene polymorphisms and the postoperative pulmonary infection was observed.RESULTS The Fas/Fasl genotypes of the two groups of patients were in accordance with the law of genetic equilibrium;the frequencies of A/A genotype,allele A at rs1571013 locus and G/G genotype,allele G at rs1800682 locus were significantly higher in the infection group than in the no infection group(P<0.05).There were no significant differences in the genotypes and alleles at rs6700734 locus and rs763110 locus of Fasl gene between the two groups.The patients with A/A genotype and allele A at rs1571013 locus were at higher risk of postoperative pulmonary infection than the patients with G/G genotype and allele G(P<0.05);the patients with G/G genotype,A/G genotype and allele G at rs1800682 locus were at higher risk of postoperative pulmonary infection than the patients with A/A genotype and allele A(P<0.05).CONCLUSION The rs1571013 locus and rs1800682 locus of Fas gene are associated with the postoperative pulmonary infection in the patients with acute myocardial infarction,and the monitoring of genotypes and related loci may facilitate the clinical prediction of postoperative pulmonary infection.
