摘要
目的探究癌胚抗原(CEA)、血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)与肺癌并发肺部感染的相关性。方法选取2018年5月-2021年4月枣庄市立二院收治的60例肺癌并发肺部感染患者为研究组,另选取同期医院就诊未发生肺部感染肺癌患者60例为对照组,分析两组患者血清CEA、VEGF、MMP-9表达水平及其与临床肺部感染(CPIS)评分的相关性,绘制受试者工作特征(ROC)曲线,评估CEA、VEGF、MMP-9表达对肺癌并发肺部感染的诊断价值。结果研究组患者血清CEA(16.69±4.06)U/ml、VEGF(766.21±51.54)pg/ml、MMP-9(99.28±15.26)ng/ml表达水平均高于对照组(P<0.05);CEA、VEGF、MMP-9表达与CPIS评分呈正相关关系(P<0.05);ROC曲线分析结果显示,CEA、VEGF、MMP-9联合检测诊断肺癌并发肺部感染的曲线下面积(AUC)为0.909,敏感度为76.67%,特异度为90.00%。结论肺癌并发肺部感染患者CEA、VEGF、MMP-9呈高表达,临床可通过检测其表达,早期诊断,及时采取有效防治措施,有助于改善患者预后。
OBJECTIVE To explore the association between carcinoembryonic antigen(CEA),vascular endothelial growth factor(VEGF),matrix metalloproteinase-9(MMP-9)and pulmonary infection in the lung cancer patients.METHODS Totally 60 lung cancer patients who were complicated with pulmonary infection and treated in Zaozhuang Municipal Second Hospital from May 2018 to Apr 2021 were assigned as the study group.meanwhile,60 lung cancer patients who did not have pulmonary infection were chosen as the control group.The association of the expression levels of serum CEA,VEGF and MMP-9 with clinical pulmonary infection score(CPIS)was observed.The values of CEA,VEGF and MMP-9 in diagnosis of pulmonary infection were evaluated by means of receiver operating characteristic(ROC)curves.RESULTS The expression levels of serum CEA,VEGF and MMP-9 of the study group were respectively(16.69±4.06)U/ml,(766.21±51.54)pg/ml and(99.28±15.26)ng/ml,higher than those of the control group(P<0.05).The expression levels of CEA,VEGF and MMP-9 were positively correlated with CPIS(P<0.05).ROC curve analysis showed that the area under curve(AUC)of the joint detection of CEA,VEGF and MMP-9 was 0.909 in diagnosis of pulmonary infection in the lung cancer patients,with the sensitivity 76.67%,the specificity 90.00%.CONCLUSION The lung cancer patients complicated with pulmonary infection show high expressions of CEA,VEGF and MMP-9.The clinical detection of the indexes may facilitate the early diagnosis so as to take effective prevention measures in a timely manner and improve prognosis of the patients.
