免疫分子B7-H3在喉声带白斑组织的表达及临床意义

摘要 目的 探讨新型免疫分子B7-H3在喉声带白斑不同病理阶段的表达及可能的临床意义。方法 使用免疫组化分析B7-H3在5种喉白斑病理型包括单纯上皮增生、轻度上皮异形增生、中度异形上皮增生、重度异形上皮增生(原位癌)及浸润癌的表达。结果 在收集的90例喉白斑组织中,免疫组化分析发现B7-H3主要表达于鳞状上皮细胞特别是正在增殖的细胞;其中单纯上皮增生、轻度上皮异形增生和中度异形上皮增生组织其表达水平比较低。然而,在重度异形上皮增生(原位癌)及浸润癌的标本中,B7-H3的表达水平显著上调。结论 B7-H3可高表达于炎症加剧的喉白斑及浸润癌组织的鳞状上皮细胞,提示上皮表达的B7-H3可能是一个有意义的预后预测指标和癌前病变向恶性转化的危险性参考指标。 This study was designed to investigate the expression of novel immune molecule B7-H3 in laryngeal vocal cord leukoplakia(LL) at different pathological stages and further analysis of its possible clinical significance. The expression of B7-H3 antigen in LL tissues, which was divided into five pathological subtypes including simple epithelial hyperplasia, mild dysplasia, moderate dysplasia, severe dysplasia(carcinoma in situ) and invasive carcinoma, was analyzed by immunohistochemistry. In 90 cases of LL tissues, immunohistochemical analysis showed that B7-H3 was mainly expressed in squamous epithelial cells, especially in proliferating cells. The expression level of B7-H3 was lower in simple epithelial hyperplasia, mild epithelial dysplasia and moderate epithelial dysplasia. However, it was significantly up-regulated in the specimens of severe dysplasia(carcinoma in situ) and invasive carcinoma. B7-H3 is highly expressed in laryngeal leukoploma and squamous cells of infiltrating carcinoma tissues, suggesting that epithelial B7-H3 may be a meaningful prognostic indicator and a risk reference index for malignant transformation of precancerous lesions.
