小针刀联合芍药甘草汤加减治疗痉挛性斜颈的临床观察

Clinical Observation of Acupotomy Combined with Modified Shaoyao Gancao Decoction on the Treatment of Spasmodic Torticollis

【目的】观察小针刀联合芍药甘草汤加减治疗痉挛性斜颈的临床疗效。【方法】将72例痉挛性斜颈患者随机分为观察组和对照组,每组36例,观察组给予小针刀配合芍药甘草汤加减治疗,对照组给予常规针刺配合芍药甘草汤加减治疗。4周为1个疗程,共治疗3个疗程。治疗3个月后,评价2组临床疗效,观察2组患者治疗前后Tsui评分的变化情况,以及生活质量评估量表(CDQ-24)的情况。比较2组患者治疗前后焦虑自评量表(SAS)评分的变化情况。【结果】(1)治疗后,2组患者的Tsui评分明显改善(P<0.05),且观察组在改善Tsui评分方面明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的CDQ-24评分明显改善(P<0.05),且观察组在改善CDQ-24评分方面明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,2组患者的SAS评分明显改善(P<0.05),且观察组在改善SAS评分方面明显优于对照组,差异有统计学意义(P<0.05)。(4)观察组总有效率为94.44%(34/36),对照组为75.00%(27/36)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。【结论】小针刀联合芍药甘草汤加减治疗痉挛性斜颈,能明显改善患者的临床症状,有效改善患者的焦虑状态,提高患者的生活质量,疗效显著。

Objective To observe the clinical efficacy of acupotomy combined with modified Shaoyao Gancao Decoction(composed of Paeoniae Alba Radix and Glycyrrhizae Radix et Rhizoma mainly with the actions of regulating the function of the liver and spleen,and soften tendons to relieve pain)on the treatment of spasmodic torticollis(ST).Methods The 72 patients with ST were randomly divided into the observation group and the control group,36 patients in each group.The observation group was treated with acupotomy combined with modified Shaoyao Gancao Decoction,while the control group was treated with conventional acupuncture with modified Shaoyao Gancao Decoction.Four weeks as a course of treatment,a total of 3 courses of treatment were performed.After 3 months of treatment,the clinical efficacy of the two groups was evaluated,the Craniocervical Dystonia Questionnaire(CDQ-24)and the change in Tsui score were observed before and after treatment.The changes in self-rating anxiety scale(SAS)scores were compared before and after treatment between the two groups.Results(1)After treatment,the Tsui scores of the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the Tsui scores,the difference being statistically significant(P<0.05).(2)After treatment,the CDQ-24 scores of the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the CDQ-24 scores,the difference being statistically significant(P<0.05).(3)After treatment,the SAS scores of the two groups were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the SAS scores,the difference being statistically significant(P<0.05).(4)The overall effective rate of the observation group was 94.44%(34/36)and that of the control group was 75.00%(27/36).The efficacy of the observation group was superior to that of the control group,the difference being statistically significant(P<0.05).Conclusion Acupotomy combined with modified Shaoyao Gancao Decoction in the treatment of ST can significantly improve the clinical symptoms of patients,effectively improve their anxiety state and enhance their quality of life with remarkable efficacy.
