胃苏颗粒联合质子泵抑制剂治疗消化性溃疡疗效的Meta分析

Meta-analysis on Weisu Granules with Proton Pump Inhibitor in Treatment of Peptic Ulcer

目的:对胃苏颗粒联合质子泵抑制剂(PPI)治疗消化性溃疡(PU)的疗效进行评估。方法:通过检索知网、万方、维普、PubMed、Cochrane library等数据库,经质量评价纳入与胃苏颗粒联合PPI治疗消化性溃疡相关的随机对照试验(RCT),筛选文献并提取相关数据,利用RevMan 5.3软件对临床有效率、胃镜有效率、不良反应发生率、幽门螺旋杆菌(HP)阳性率、症状积分进行Meta分析。结果:17篇相关文献纳入本研究,共计1850例患者,Meta分析结果提示:临床有效率(OR=6.75,95%CI [3.89,11.71],P<0.00001);胃镜有效率(SMD=-5.40,95%CI [-7.51,-3.30],P<0.00001);不良反应发生率(OR=0.47,95%CI[0.32,0.68],P<0.0001);HP阳性率(OR=0.17,95%CI [0.10,0.28],P<0.00001);症状积分(SMD=-9.24,95%CI [-12.24,-6.23],P<0.00001)。结论:与单纯使用PPI相比,联合使用胃苏颗粒能够有效提高临床治疗消化性溃疡的疗效,主要体现在实验组临床有效率和胃镜下溃疡愈合率较对照组效果更好,不良反应发生率、HP阳性率和症状积分发生机会较对照组更低,但受纳入文献质量的影响,尚需更多高质量的研究以增加证据强度。

ObjectiveTo evaluate the efficacy of Weisu granules combined with proton pump inhibitors(PPI) in the treatment of peptic ulcer(PU). Methods Randomized controlled trials(RCTs) related to the treatment of peptic ulcer with Weisu granules combined with PPI were included by quality evaluation through searching databases such as CNKI, Wanfang, Vipshop, PubMed and Cochrane library, screening the literature and extracting relevant data, and using RevMan 5.3 software to analyze the clinical efficiency, gastroscopic efficiency, incidence of adverse effects, Helicobacter pylori(HP) positivity rate and symptom score.Results Seventeen relevant papers were included in this study, with a total of 1850 patients. The results of the Meta-analysis suggested that the clinical efficiency(OR=6.75,95%CI [3.89,11.71],P<0.00001);gastroscopic efficiency(SMD=-5.40,95%CI [-7.51,-3.30],P<0.00001);adverse effects incidence(OR=0.47,95%CI [0.32,0.6],P<0.00001);HP positive rate(OR=0.17,95%CI [0.10,0.28],P<0.00001);symptom score(SMD=-9.24,95%CI [-12.24,-6.23],P<0.00001).Conclusion Compared with PPI alone, the combination of Weisu granules was effective in improving the clinical efficacy of peptic ulcer treatment, mainly in terms of better clinical efficiency and gastroscopic ulcer healing rate in the experimental group compared with the control group, and lower incidence of adverse effects, HP positive rate and chance of symptom score compared with the control group, but more high-quality studies are needed to increase the strength of evidence due to the quality of the included literature.
