改良吻合器痔上黏膜环切术联合吻合口缝合加固治疗Ⅲ~Ⅳ期混合痔的疗效观察

Clinical effects of procedure for prolapse and hemorrhoids combined with anastomotic suture reinforced treated with stageⅢ–Ⅳmixed hemorrhoid

目的探讨吻合器痔上黏膜环切术联合吻合口缝合加固治疗Ⅲ~Ⅳ期混合痔的疗效。方法选取2020年5月至2022年4月中国人民解放军联勤保障部队第九〇〇医院仓山院区收治的112例Ⅲ~Ⅳ期混合痔患者为研究对象,根据随机数字表法将患者分成观察组和对照组,每组各56例。对照组患者采用传统痔疮手术治疗;观察组患者采用吻合器痔上黏膜环切术联合吻合口缝合加固治疗。比较两组患者的手术时间、住院时间、术中出血量、术后并发症发生率。结果观察组患者的手术时间、住院时间显著短于对照组,术中出血量显著少于对照组(P<0.05);观察组患者的术后并发症总发生率显著低于对照组(3.56%vs.23.21%,P<0.05)。结论吻合器痔上黏膜环切术联合吻合口缝合加固治疗混合痔可缩短手术时间、住院时间,减少术中出血量,降低术后并发症发生率,值得临床推广。

Objective To analyze the clinical effects of procedure for prolapse and hemorrhoids combined with anastomotic suture reinforced treated with stageⅢ-Ⅳmixed hemorrhoid.Methods A total of 112 patients with stageⅢ-Ⅳmixed hemorrhoids in Joint Logistics Support Force 900th Hospital Cangshan Hospital Area of PLA from May 2020 to April 2022 were selected as the research objects.According to random number table method,the patients were divided into observation group and control group,with 56 cases in each group.The control group was treated with traditional hemorrhoidectomy;the observation group was treated with procedure for prolapse and hemorrhoids combined with anastomotic suture reinforced.Operative time,length of hospital stay,intraoperative blood loss,and postoperative complication rate were compared between the two groups.Results The operation time and hospital stay in observation group were significantly shorter than those in control group,and the intraoperative blood loss was significantly less than that in control group(P<0.05).The total incidence of postoperative complications in observation group was significantly lower than that in control group(3.56%vs.23.21%,P<0.05).Conclusion The procedure for prolapse and hemorrhoids combined with anastomotic suture reinforcement for the treatment of mixed hemorrhoids can shorten the operation time and hospital stay,reduce the amount of intraoperative bleeding,and the incidence of postoperative complications,which is worthy of clinical promotion.
