多层螺旋CT对老年桡骨远端骨折分型的诊断研究

Diagnosis of distal radius fractures in the elderly with multislice spiral computed tomography

目的分析多层螺旋CT(multislice spiral computed tomography,MSCT)对老年桡骨远端骨折分型的诊断价值。方法选取2018年1月至2020年9月义乌市中心医院收治的80例桡骨远端骨折患者为研究对象,对其分别行X线检查和MSCT检查。比较两种检查的图像特征,并以手术结果作为金标准,分析两种检查对骨折分型诊断的符合率。结果MSCT检查测量患者的冠状面腕关节面塌陷度、矢状面骨折位移程度和骨折块数均显著高于X线检查(P<0.05);MSCT检查的桡骨远端骨折分型与手术分型的符合率高于X线检查(P<0.05);MSCT扫描检查多可见压缩性塌陷,骨皮质不连续,多个骨折块,腕关节间隙变窄。结论MSCT可提高老年桡骨远端骨折患者的分型诊断符合率。

Objective To analyze the diagnostic value of multislice spiral computed tomography(MSCT)in the classification of distal radius fractures in the elderly.Methods A total of 80 patients with distal radius fracture admitted to Yiwu Central Hospital from January 2018 to September 2020 were selected as the study subjects.X-ray examination and MSCT examination were performed on them,respectively.The image features of the two tests were compared,and the coincidence rate of fracture classification was analyzed with surgical results as the gold standard.Results The collapse degree of the coronal wrist surface,the displacement degree of sagittal fracture and the number of fractures measured by MSCT were significantly higher than those measured by X-ray examination(P<0.05).The coincidence rate of distal radius fracture classification by MSCT was higher than that by X-ray(P<0.05).On MSCT scan,compression collapse,cortical discontinuity,multiple fractures,and narrowing of the wrist space were observed.Conclusion MSCT can improve the classification coincidence rate in elderly patients with distal radius fractures.
