多模态超声技术评分法在脑卒中预测中的研究

Study of multimodal ultrasound technology scoring method in stroke prediction

目的通过多种超声技术对脑卒中相关危险因子进行检查,采用多模态超声技术评分预测脑卒中发生的可能性。方法选取2019年6月至2020年12月于绍兴市柯桥区中医医院就诊的298例患者和125例健康体检人员,按临床症状分为3组,第1组为健康人员(n=125),第2组为有高血压、高血脂、糖尿病1项以上,或有头晕、头痛等疑似脑卒中先兆症状的患者(n=189),第3组为有临床症状且有脑卒中影像学依据的患者(n=109),对所有研究对象进行多种超声检查并对各项检查结果进行评分。结果比较3组的颈动脉斑块、颈动脉狭窄、心脏、后循环超声情况以及综合后的超声分值情况,结果显示各项第1组均优于第2组和第3组,差异有统计学意义(P<0.05)。结论颈动脉斑块、颈动脉狭窄、心脏超声改变对脑卒中发生具有预测及评价意义,评分越高脑卒中发生的可能性越高,多模态超声技术评分法可以作为对脑卒中发生的预测方法并应用于临床,作为评估脑卒中发生的一种综合性超声分级的评价方法。

Objective To predict the likelihood of stroke using a multimodal ultrasound technique score,which examines risk factors associated with stroke through a variety of ultrasound techniques.Methods A total of 298 patients and 125health check-ups attending the Shaoxing City Keqiao District Hospital of Traditional Chinese Medicine from June 2019 to December 2020 were selected and divided into three groups according to clinical symptoms.Group 1 was healthy(n=125),group 2 was patients with one or more of hypertension,hyperlipidemia,diabetes mellitus,or suspected stroke aura symptoms such as dizziness and headache(n=189),and group 3 was patients with clinical symptoms and imaging evidence of stroke(n=109).Multiple ultrasound examinations were performed on all study subjects and the results of each examination were scored.Results Compared the carotid plaque,carotid stenosis,cardiac and posterior circulation ultrasound conditions and the combined ultrasound scores of the three groups,the results showed that each group 1 was superior to groups 2 and 3,with statistically significant differences(P<0.05).Conclusion Carotid plaque,carotid stenosis and cardiac ultrasound changes have predictive and evaluative significance for stroke occurrence.The higher the score,the higher the likelihood of stroke occurrence.The multimodal ultrasound technique score can be used as a predictive method for stroke occurrence and applied clinically as a comprehensive ultrasound grading evaluation method for assessing stroke occurrence.
