摘要
目的 调查分析某院6种治疗药物的血清药物浓度监测情况,探讨血清药物浓度监测的临床价值,促进临床个体化治疗。方法收集某院2020年1月~2020年6月共243份血清药物浓度监测结果并统计分析。结果 243份样本中丙戊酸钠送检样本最多,占比34.16%;卡马西平样本有88.89%控制在有效血清药物浓度范围内;地高辛样本有83.33%血清药物浓度偏低;万古霉素样本有53.82%谷浓度在正常参考值范围内,环孢素样本有47.62%谷浓度100~200 ng/ml;监测甲氨蝶呤大剂量冲击疗法后24 h、48 h、72 h血清药物浓度,有28例延迟排泄。结论 某院的血清治疗药物监测需提高送检样本量,加强动态监测,从而减少药物毒性反应,提高疗效。
Objective To investigate and analyze the monitoring of serum drug concentration of six therapeutic drugs in our hospital, and to explore the clinical value of serum drug concentration monitoring, so as to promote clinical individualized treatment. Methods A total of 243serum drug concentration monitoring results from January 2020 to June 2020 in our hospital were collected and statistically analyzed. Results Among the 243 samples, sodium valproate was the most, accounting for 34.16%. 88.89% of carbamazepine samples were controlled within the range of effective serum drug concentration;83.33% of digoxin samples had low serum drug concentration;53.82% trough concentration of vancomycin was within the normal reference value range, and 47.62% trough concentration of cyclosporine was 100 ~ 200 ng/ml. After monitoring the serum drug concentration of methotrexate at 24 h, 48 h and 72 h after high-dose pulse therapy, there were 28 cases with delayed excretion. Conclusion In the monitoring of serum therapeutic drugs in our hospital, it is necessary to increase the sample quantity and strengthen the dynamic monitoring, so as to reduce the drug toxicity and improve the curative effect.
