摘要
目的探索与优化全自动药品单剂量分包模式下的药品管理。方法采取多重措施控制和监控药品分包环境的湿度;选用塑料乐扣盒作为储药盒,在其外侧标明药盒编号、药品名称、规格、数量、生产批号、原包装有效期、拆零日期及拆零后有效期,内侧粘贴一粒供核对药片实样,对预剥药进行有效管理;按照药品名称的拼音首字母制作储药盒编号目录,结合储药盒位置分布表,快速确定某个药品储药盒的位置;在HIS建立整片药消耗量查询、统计模块,确定并定期调整每个药品的合适预剥药数量,通过定位定量双瓶轮换机制管理非整片药,并制作位置分布表便于药师快速定位非整片药;自动调用包药机内药品库存量生成分站点盘存数据,以提高盘存工作效率;通过在HIS中切换不同院区的包药机,建立利用其他院区机器进行自动分包的应急机制;对性质不稳定药品采取特殊处理,以确保分包药品的质量。结果整片药和非整片药的质量和有效期得到了有效管理,80%以上药品的预剥药数量控制在2周消耗量的50%~150%,提高了包药机内药品盘存的工作效率,建立起了利用其他院区包药机进行自动分包的应急机制。结论在全自动药品单剂量分包模式下对药品管理进行有针对性的优化与创新,可确保药品分包工作的质量与效率。
Objective To explore and optimize drug management mode in automated single-dose drug dispensing and packing.Methods Multiple measures were taken to control and monitor the humidity in the automated dispensing and packing cabin.The LOCK&LOCK plastic boxes were selected as the drug storage boxes,with drug′s serial number,name,specification,quantity,batch number,original valid date,unpackaging date,and valid date after unpackaging being indicated on two outer sides of each box,a sample tablet being fixed on one inner side of each box for checking,thereby managing the unpackaged drugs effectively.In combination with tables for location distribution of the drug storage boxes,a directory of boxes′serial numbers was made according to drug′s initial name of pinyin for quick spotting.A statistical module to query the consumption of the intact tablets was set up in the hospital information system(HIS),and the appropriate unpackaging quantity for each drug was determined and adjusted at a regular interval.A fixed-position,fixed-quantity,double-bottle rotation mechanism was established to manage the split tablets,with a sheet of location distribution being made for pharmacists to quickly spot the split tablets.The amounts of the drugs in the machine were automatically obtained from the machine′s APPS as a subset of the inventory data to improve the efficiency of inventory taking.By switching in a HIS module among machines in different areas of our hospital,the emergency mechanism for automated dispensing and packing was established.Finally,a special treatment was adopted to ensure the quality of an unstable drug.Results The quality and valid dates of the intact and split tablets were under effective control,more than 80%drugs had ratios of unpackaging quantity within 50%~150%of two-week consumption,the inventory-taking efficiency for drugs in the machine was enhanced,and an emergency mechanism for automated dispensing and packing was established by using machines in different areas of our hospital.Conclusion The quality and efficiency automated dispensing and packing of single-dose drug can be well ensured through optimization and innovation of drug management mode.
