自拟钩草平肝汤联合穴位贴敷治疗椎基底动脉供血不足性眩晕的临床效果

Clinical effect of self-made Goucao Pinggan Decoction combined with acupoint sticking in the treatment of vertigo due to vertebrobasilar insufficiency

目的探讨自拟钩草平肝汤联合穴位贴敷治疗椎基底动脉供血不足性眩晕的临床效果。方法选取2021年6月至2022年5月抚州市中医院内科收治的70例肝阳上亢型椎基底动脉供血不足性眩晕患者为研究对象,采用随机数字表法将患者分为对照组、观察组各35例,对照组采用盐酸倍他司汀注射液静脉输液治疗,观察组在对照组的基础上加自拟钩草平肝汤口服及中药穴位贴敷治疗,疗程为10 d。比较两组患者治疗前后经颅多普勒(TCD)血流检查指标及中医证候疗效。结果治疗前,两组患者的左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)的平均血流速度(V_(m))比较,差异无统计学意义(P>0.05);治疗后,观察组的LVA、RVA、BA的V_(m)高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者的LVA、RVA、BA的V_(m)均高于本组治疗前,差异有统计学意义(P<0.05);观察组的中医证候疗效优于对照组,差异有统计学意义(P<0.05);观察组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论自拟钩草平肝汤联合穴位贴敷可以提高椎基底动脉的平均血流速度,改善患者的血液循环,从而有效提高椎基底动脉供血不足性眩晕的治疗有效率。

Objective To investigate the clinical effect of self-made Goucao Pinggan Decoction combined with acupoint sticking on vertebral-basilar insufficiency vertigo.Methods A total of 70 patients with vertebrobasilar insufficiency vertigo of liver-yang hyperactivity type treated in the Department of Internal Medicine,Fuzhou Hospital of Traditional Chinese Medicine from June 2021 to May 2022 were selected as the research objects,and they were divided into the control group and the observation group by random number table method,with 35 cases in each group.The control group was treated with Betastine Hydrochloride Injection and intravenous infusion,and the observation group was additionally treated with self-made Goucao Pinggan Decoction and acupoint application of traditional Chinese medicine on the basis of the control group.The course of treatment was 10 days.The indexes of transcranial Doppler(TCD)blood flow examination before and after treatment and the efficacy of traditional Chinese medicine syndrome were compared between the two groups.Results Before treatment,there were no significant differences in the mean blood flow velocity(V_(m))of the left vertebral artery(LVA),right vertebral artery(RVA)and basilar artery(BA)between the two groups(P>0.05).After treatment,the V_(m)of LVA,RVA and BA in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the V_(m)of LVA,RVA and BA in the two groups were higher than those before treatment,and the differences were statistically significant(P<0.05).The traditional Chinese medicine syndrome effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Self-made Goucao Pinggan Decoction combined with acupoint application can increase the average blood flow velocity of the vertebral-basilar artery and improve the blood circulation of patients,thereby effectively improving the treatment efficiency of vertigo of vertebral-basilar insufficiency.
