500张糖尿病患者门诊处方的用药分析及点评

Analysis and Comment of 500 Outpatient Prescriptions for Diabetic Patients

目的:分析及点评500张糖尿病患者门诊处方的用药规律。方法:随机选取2022年1-8月广州医科大学附属第二医院诊断为糖尿病的门诊处方500张,对单次就诊处方所用药品数量、种类及不合理情况,进行分析。结果:500张门诊处方中,1种药品占21.80%;2种药品占25.60%;3种药品占30.40%;4种药品占16.60%;≥5种药品占5.60%。药物使用频率前5位依次为二甲双胍、达格列净、罗格列酮、吡格列酮、恩格列净。不合理处方中,存在不规范处方、不适宜处方及超常处方的相关问题,不适宜处方所占比例较高。其中,用法、用量及适应证不适宜占不适宜处方比例最高;超常处方中,无适应证及超说明书处方均存在。结论:在糖尿病患者500张门诊处方中,药品齐全,多药联用较为普遍,与用药指南基本一致,不合理处方占比少,合理性较高。在后续的临床工作中,可对不适宜处方进行强化管理,从而合理用药。

Objective:To analyze and comment on 500 outpatient prescriptions of diabetic patients.Methods:A total of 500 outpatient prescriptions diagnosed as diabetes in the Second Affiliated Hospital of Guangzhou Medical University from January to August 2022 were randomly selected.The quantity,type and irrationality of drugs used in single visit prescription were analyzed.Results:Among 500 outpatient prescriptions,21.80%were of one drug;two drugs accounted for 25.60%;three drugs accounted for 30.40%;four drugs accounted for 16.60%;≥5 drugs accounted for 5.60%.The top five drug use frequency were metformin,dapagliflozin,rosiglitazone,pioglitazone,and empagliflozin.Among irrational prescriptions,there were some problems related to non-standard prescription,inappropriate prescription and abnormal prescription,and the proportion of inappropriate prescription was relatively high.Among them,usage,dosage and indications accounted for the highest proportion of inappropriate prescriptions.There were no indications and off-label prescriptions in the supernormal prescriptions.Conclusion:Among 500 outpatient prescriptions for diabetic patients,the drugs were complete and multi-drug combination was common,which was basically consistent with the medication guidelines.Irrational prescriptions accounted for less and higher rationality.In the follow-up clinical work,the management of inappropriate prescription can be strengthened,to rationalize drug use.
