眼底激光联合曲安奈德治疗糖尿病视网膜病变的临床疗效

Clinical Efficacy of Fundus Laser Combined with Triamcinolone Acetonide in the Treatment of Diabetic Retinopathy

目的:分析眼底激光联合曲安奈德治疗糖尿病视网膜病变的临床疗效。方法:选取2019年3月-2022年2月甘肃省白银爱尔眼科医院收治的糖尿病视网膜病变患者40例为观察对象,根据随机数字表法分成对照组与观察组,各20例(20眼)。对照组实施眼底激光治疗,观察组在对照组基础上联合曲安奈德辅助治疗。比较两组患者最佳矫正视力评分(BCVA)、黄斑区视网膜厚度,血清血管内皮生长因子(VEGF)、细胞间黏附分子(ICAM-1)水平,疾病症状缓解时间,视力提高率,不良反应发生率。结果:治疗后,两组患者BCVA评分与黄斑区视网膜厚度均降低,观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者VEGF水平与ICAM-1水平均降低,观察组低于对照组,差异有统计学意义(P<0.05);观察组患者视网膜水肿缓解时间和眼底渗出吸收时间均短于对照组,差异有统计学意义(P<0.05);观察组视力提高率高于对照组,差异有统计学意义(P<0.05)。结论:糖尿病视网膜病变患者应用曲安奈德联合眼底激光治疗,可显著改善患者视力和黄斑区视网膜厚度,改善VEGF水平与ICAM-1水平。

Objective:To analyze the clinical efficacy of fundus laser combined with triamcinolone acetonide in the treatment of diabetic retinopathy.Methods:40 patients with diabetic retinopathy admitted to Baiyin Aier Eye Hospital from March 2019 to February 2022 were selected as observation subjects and divided into control group and observation group according to the random number table method,20 cases(20 eyes)each.The control group was treated with fundus laser therapy,and the observation group was treated with triamcinolone acetonide on top of the control group.The best corrected visual acuity score(BCVA),macular retinal thickness,serum vascular endothelial growth factor(VEGF)and intercellular adhesion molecule(ICAM-1)levels,time to remission of disease symptoms,visual acuity improvement rate,and incidence of adverse effects were compared between the two groups.Results:After treatment,the BCVA score and macular retinal thickness decreased in both groups,and the observation group was lower than the control group.The difference was statistically significant(P<0.05).After treatment,VEGF levels and ICAM-1 levels were reduced in both groups,and the observation group was lower than the control group.The difference was statistically significant(P<0.05).The time of retinal edema relief and fundus exudate absorption were shorter in the observation group than in the control group,and the difference was statistically significant(P<0.05).The improvement rate of visual acuity in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of triamcinolone acetonide combined with fundus laser treatment in patients with diabetic retinopathy can significantly improve patients'visual acuity and macular retinal thickness,and improve VEGF levels and ICAM-1 levels.
