摘要
目的探讨血清巨噬细胞迁移抑制因子(MIF)、LRRK2表达联合帕金森病综合评分量表(UPDRS)评分对帕金森病的诊断价值及其与疾病分期的关系分析。方法前瞻性选取2019年7月至2022年8月南通市第一人民医院收治的100例帕金森病患者作为研究对象,纳入研究组,并纳入同期年龄、性别配对的100名体检健康者作为对照,纳入对照组。比较帕金森病患者和对照组的外周血MIF、LRRK2的表达;采用Logistics回归模型分析帕金森病发生的独立危险因素;明确MIF mRNA、LRRK2 mRNA和UPDRS评分诊断帕金森病的临床价值;分析MIF mRNA、LRRK2 mRNA、UPDRS评分与帕金森病疾病分期的相关性。结果帕金森病患者MIF mRNA、LRRK2 mRNA、UPDRS评分均显著高于对照组,差异均有统计学意义(P<0.05),且随着疾病分期的增加MIF mRNA、LRRK2 mRNA、UPDRS评分亦明显更高(P<0.05)。多因素分析结果显示,MIF mRNA、LRRK2 mRNA、UPDRS评分是影响帕金森病发生的独立危险因素(OR=1.954,95%CI=1.354~3.564;OR=2.364,95%CI=1.523~4.654;OR=2.575,95%CI=1.654~5.654,P<0.05)。MIF mRNA、LRRK2 mRNA联合UPDRS评分诊断帕金森病的敏感度和特异度达到93.54%和85.45%,曲线下面积为0.897;MIF mRNA、LRRK2 mRNA、UPDRS评分的表达均与帕金森病患者病情分期密切相关(r=0.49、0.52、0.54,P<0.05)。结论MIF和LRRK2的高表达可作为诊断帕金森病的可靠血清标志物,且可检测MIF和LRRK2的表达及联合UPDRS评分对帕金森病疾病分期做出可靠评估。
Objective To explore the diagnostic value of serum macrophage migration inhibitory factor(MIF)and LRRK2 expression combined with Unified Parkinson’s Disease Rating Scale(UPDRS)score in Parkinson’s disease and its relationship with disease staging.Methods One hundred patients with Parkinson’s disease admitted to Nantong First People’s Hospital from July 2019 to August 2022 were prospectively selected as the research objects,included in the research group,and included age-and gender-matched healthy people as controls,and included in the control group(n=100).The expressions of MIF and LRRK2 in peripheral blood of Parkinson’s patients and control group were compared;Logistic regression model was used to analyze the independent risk factors of Parkinson’s disease;the clinical value of MIF mRNA,LRRK2 mRNA and UPDRS scores in the diagnosis of Parkinson’s disease was determined;the correlation between MIF mRNA,LRRK2 mRNA,UPDRS score and Parkinson’s disease stage was analyzed.Results The scores of MIF mRNA,LRRK2 mRNA and UPDRS in Parkinson’s patients were higher than those in the control group(P<0.05),and the scores of MIF mRNA,LRRK2 mRNA and UPDRS were also significantly higher with the increase of disease stage(P<0.05).The results of multivariate analysis showed that MIF mRNA,LRRK2 mRNA and UPDRS score were independent risk factors for the occurrence of Parkinson’s disease(OR=1.954,95%CI=1.354-3.564,OR=2.364,95%CI=1.523-4.654;OR=2.575,95%CI=1.654-5.654,P<0.05).The sensitivity and specificity of MIF mRNA,LRRK2 mRNA combined with UPDRS score in the diagnosis of Parkinson’s disease reached 93.54%and 85.45%,area under curve value was 0.897;the expressions of MIF mRNA,LRRK2 mRNA,and UPDRS score were closely related to the stage of Parkinson’s disease(r=0.49,0.52,0.54,P<0.05).Conclusion The high expression of MIF and LRRK2 can be used as reliable serum markers for the diagnosis of Parkinson’s disease,and the expression of MIF and LRRK2 can be detected and combined with UPDRS score to make a reliable assessment of Parkinson’s disease staging.
