家庭医生签约服务模式下全科医疗核心特征功能对患者就诊满意度的影响——以河南省J县为例

Influence of Core Features of General Practice under Family Doctor Contract on Patient Satisfaction:Take J County of Henan Province as an example

目的:从患者角度评价河南省J县家庭医生签约服务效果,分析全科医疗核心特征功能对患者就诊满意度的影响。方法:2022年8月2—11日,对河南省J县10个基层医疗卫生机构的274例患者开展问卷调查,按照是否签约家庭医生将其分为签约组(n=159)和未签约组(n=115)。结果:签约组患者全科医疗核心特征功能体验量表得分为(82.52±14.98)分,高于未签约组的(58.39±15.09)分,签约组量表各维度得分均高于未签约组;签约组就诊总体满意度得分为(93.84±12.92)分,高于未签约组的(71.13±19.86)分;除医务人员技术水平和医药费用报销比例外,签约组满意度得分均高于未签约组;多元线性回归结果显示,首诊/第一线照护、服务可及性、综合性、协调性会不同程度影响患者的就诊满意度。结论:家庭医生签约服务提升了基层医疗卫生机构的全科医疗核心特征功能,核心特征功能会影响患者的就诊满意度。

OBJECTIVE To evaluate the effect of family doctor contract service in J county from the perspective of patients, and to analyze the influence of core characteristics of general medical treatment on patients’ satisfaction. METHODS A questionnaire survey was conducted on 300 patients in 10 primary health care institutions in J County, Henan Province. RESULTS There were 159 patients in contract group and 115 patients in non-contract group. The total score of the core characteristic function of general practice in the contracted group was 82.52, which was higher than 58.39 in the non-contracted group. The scores of each dimension of the core characteristic function in the contracted group were higher than those in the non-contracted group. The overall satisfaction of the contracted group was 93.84, higher than that of the non-contracted group 71.13. Except for the technical level of medical staff and the proportion of medical expenses reimbursement, the satisfaction scores of the contracted group were higher than those of the non-contracted group. The results of multiple linear regression showed that the first diagnosis, accessibility, comprehensiveness and coordination would affect the satisfaction of patients to varying degrees. CONCLUSION Family doctor contract improves the core characteristics of patients’ general practice, which will affect patients’ satisfaction.
