Siebens领域管理模式为基础的延续性护理对心房颤动患者临床结局的影响

Effect of continuous nursing based on Siebens domain management model on clinical outcome of patients with atrial fibrillation

目的探讨Siebens领域管理模式(SDMM)为基础的延续性护理对心房颤动患者临床结局的影响。方法选择2020年6月-2021年8月在华中科技大学同济医学院附属协和医院接受治疗的心房颤动患者312例作为研究对象。依据患者的入院时间将2020年6月-2021年1月入院的患者作为对照组(n=156),将2021年2-8月入院的患者作为观察组(n=156)。对照组实施常规的延续性护理,观察组实施基于SDMM的延续性护理。比较2组患者生活质量、用药依从性和再入院率。结果出院6个月,观察组患者的生活质量各项评分均明显高于对照组(P<0.05);观察组患者的服药依从性明显优于对照组(P<0.05)。在出院后6个月的时间内,观察组的再入院率低于对照组(P<0.05)。结论在心房颤动患者的治疗以及出院康复过程中,采取SDMM为基础的延续性护理工作模式,有助于保证患者的用药依从性,提升患者生活质量,降低再入院率,值得临床推广。

Objective To explore the effect of continuous nursing based on Siebens domain management model(SDMM)on the clinical outcome of patients with atrial fibrillation.Methods A total of 312 patients with atrial fibrillation treated in Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from June 2020 to August 2021 was selected as the research object.Patients were grouped according to the time of admission,of which those admitted from June 2020 to January 2021 were the control group(n=156),and those admitted from February to August 2021 were the observation group(n=156).The control group received routine continuous nursing,and the observation group adopted continuous nursing based on SDMM.The quality of life,medication compliance,and readmission rate between the two groups were compared.Result Six months after discharge,the quality of life scores of the observation group in each dimension were significantly higher than those of the control group(P<0.05);The medication compliance of observation group was significantly better than that of control group(P<0.05).The readmission rate in the observation group was lower than that in the control group at 6 months after discharge(P<0.05).Conclusion In the treatment of patients with atrial fibrillation and the process of discharge and rehabilitation,the continuous nursing mode based on SDMM is helpful to ensure the medication compliance of patients,improve quality of life of patients,and reduce the readmission rate of patients,and is worthy of clinical promotion.
