骨化三醇联合替米沙坦治疗早期糖尿病肾病患者的效果

Effects of Calcitriol combined with Telmisartan in treatment of patients with early diabetic nephropathy

目的:观察骨化三醇联合替米沙坦治疗早期糖尿病肾病(DN)患者的效果。方法:选取2020年1月至2021年12月该院收治的120例早期DN患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各60例。两组均进行基础治疗,在此基础上,对照组予以替米沙坦治疗,观察组在对照组基础上联合骨化三醇治疗,两组均连续治疗12周。比较两组临床疗效、治疗前后血清学指标(脂蛋白α、脂联素、血钙、血磷)水平、肾功能指标[血肌酐(Scr)、血尿素氮(BUN)]水平,以及不良反应发生率。结果:观察组治疗总有效率为96.67%(58/60),高于对照组的86.67%(52/60),差异有统计学意义(P<0.05);治疗后,观察组血清脂蛋白α、血磷、Scr、BUN水平均低于对照组,差异有统计学意义(P<0.05);观察组脂联素、血钙水平均高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:骨化三醇联合替米沙坦治疗早期DN患者可提高疗效,改善血清学指标水平,以及降低肾功能指标水平,效果优于单纯替米沙坦治疗。

Objective:To observe effects of Calcitriol combined with Telmisartan in treatment of patients with early diabetic nephropathy(DN).Methods:A prospective study was conducted on 120 patients with early DN admitted to this hospital from January 2020 to December 2021.They were divided into control group and observation group according to the random number table method,60 cases in each.Both groups received basic treatment.On this basis,the control group was treated with Telmisartan,while the observation group was treated with Calcitriol on the basis of that of the control group.Both groups were treated for 12 weeks.The clinical efficacy,the serological index levels(serum lipoproteinα,adiponectin,serum calcium,serum phosphorus),the renal function index levels[serum creatinine(Scr),blood urea nitrogen(BUN)],and the incidence of adverse reactions were compared between the two groups.Results:After the treatment,the total effective rate of the observation group was 96.67%(58/60),which was higher than 86.67%(52/60)of the control group,and the difference was statistically significant(P<0.05).The levels of serum lipoproteinα,serum phosphorus,Scr and BUN in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of adiponectin and serum calcium in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Calcitriol combined with Telmisartan in the treatment of the DN patients can improve the therapeutic effects,improve the levels of serological indicators,and reduce the levels of renal function indicators.Moreover,it is superior to single Telmisartan treatment.
