改良粘接固位种植义齿修复在后牙缺损患者中的应用效果

Application effects of modified adhesive retention implant denture restoration in patients with posterior teeth defects

目的:观察改良粘接固位种植义齿修复在后牙缺损患者中的应用效果。方法:选取2019年5月至2020年4月该院收治的71例牙缺损患者进行前瞻性研究,按照随机数字表法分为观察组(n=36)和对照组(n=35)。观察组采用改良粘接固位种植义齿修复,对照组采用常规粘接固位种植义齿修复,比较两组改良出血指数(mSBI)等级、改良菌斑指数(mPLI)等级、牙槽骨厚度、牙槽骨高度、骨吸收量和满意度。结果:观察组mSBI、mPLI等级均低于对照组,差异有统计学意义(P<0.05);观察组舌侧、唇侧牙槽骨厚度和高度均高于对照组,远中、近中骨吸收量均少于对照组,差异有统计学意义(P<0.05);两组语言功能满意度评分比较,差异无统计学意义(P>0.05);观察组咀嚼功能、固位功能、舒适程度、美观度方面满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论:改良粘接固位种植义齿修复应用于后牙缺损患者可降低mSBI、mPLI等级,提高牙槽骨厚度、牙槽骨高度和满意度,减少骨吸收量,优于常规粘接固位种植义齿修复效果。

Objective: To observe application effects of modified adhesive retention implant denture restoration in patients with posterior teeth defects. Methods: A prospective study was conducted on 71 patients with tooth defects admitted to the hospital from May 2019 to April 2020.They were divided into observation group(n= 36) and control group(n= 35) according to the random number table method. The observation group was treated with modified adhesive retention implant denture restoration, while the control group was given conventional adhesive retention implant denture restoration. The modified bleeding index(mSBI), the modified plaque index(mPLI), the alveolar bone thickness, the alveolar bone height, the bone resorption, and the satisfaction degree were compared between the two groups. Results: The levels of mSBI and mPLI in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05). The levels of lingual and labial alveolar bone thickness and alveolar bone height of the observation group were higher than those of the control group, the distal and mesial bone absorption were less than those of the control group, and the differences were statistically significant(P<0.05). There was no significant difference in the language function satisfaction score between the two groups(P>0.05). Further, the satisfaction scores of chewing function, retention function, comfort and aesthetics in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). Conclusions: The modified adhesive retention implant denture for the patients with posterior tooth defects can reduce the mSBI and mPLI levels, improve the alveolar bone thickness, the alveolar bone height and the satisfaction,and reduce bone resorption. Moreover, it is superior to conventional adhesive retention implant denture restoration.
