摘要
目的:基于部分实性肺结节CT特征诊断侵袭性肺腺癌并为临床肺腺癌诊疗提供参考依据。方法:选择2020年4月至2021年4月我市三家医院接诊的侵袭性肺腺癌患者48例作为肺腺癌组,同期选择54例混合性磨玻璃病变患者为肺良性结节组,采用多中心试验设计比较两组患者影像学特征。结果:肺腺癌组胸部CT中边界清晰、血管聚集征、晕厚度不均、胸膜凹陷、实性肺结节≥5mm、多发、偏离中心生长及密度高于同层血管比例高于肺良性结节组(P<0.05);侵袭性肺腺癌结节呈类圆形、中心实性成分、边缘发现分叶、明显胸膜凹陷征、边缘毛刺。结论:基于部分实性肺结节胸部CT能够作为侵袭性肺腺癌的临床诊断依据。
Objective:To study the clinical application effect of the diagnosis of invasive lung adenocarcinoma based on CT features of some solid lung nodules,and to provide a reference for the clinical diagnosis and treatment of lung adenocarcinoma.Methods:48 cases of patients with invasive lung adenocarcinoma admitted to the 3 hospitals in our city from April 2020 to April 2021 were selected as the lung adenocarcinoma group.At the same time,54 cases of patients with mixed ground-glass nodular pneumonia were selected as the benign lung nodules group,and multicenter trial design was adopted.All patients were given Philips IQon Spectral spectral CT to scan the lesions,and to explore and compare the chest CT signs and imaging features of the two groups of patients.Results:The proportion of chest CT in lung adenocarcinoma group was clear,blood vessel aggregation,uneven thickness of halo,pleural depression and the proportion of solid lung nodules≥5mm,multiple,off-center growth and density higher than the same layer of blood vessels in the lung adenocarcinoma group was higher than that in the benign lung nodules group,the difference was statistically significant(P<0.05);Invasive lung adenocarcinoma nodules are round,and solid components can be seen in the center,lobes are found at the edges,and there are obvious signs of pleural depression;most nodules are composed of solid components,And obvious burr features are found on the edge.Conclusion:Chest CT based on some solid lung nodules can effectively identify invasive lung adenocarcinoma,which can be used as a basis for clinical diagnosis of invasive lung adenocarcinoma.
