益气通络汤联合推拿对糖尿病足创面的影响

Effect of Yiqi Tongluo Decoction combined with massage on the wound recovery in diabetic foot patients

目的探讨益气通络汤联合推拿对糖尿病足患者创面恢复情况的影响。方法将96例糖尿病足溃疡患者随机分为2组,每组各48例。2组均予以内科基础治疗,此外,对照组行常规药物及创面治疗,观察组在对照组基础上,另加益气通络汤联合推拿治疗,比较2组临床疗效,血糖代谢指标糖化血红蛋白(Hb A1c)、空腹血糖(FPG)、餐后2h血糖(2h FPG)水平,血清炎性因子C反应蛋白(CRP)、细胞坏死因子α(TNF-α)水平,以及创面恢复情况。结果观察组患者的治疗总有效率(93.75%vs.79.17%)高于对照组(P<0.05)。治疗后,观察组Hb A1c、FPG、2h FPG水平均低于对照组(P<0.05)。治疗后,观察组CRP、TNF-α水平均低于对照组(P<0.05);治疗前,2组溃疡面积比较无统计学意义(P>0.05),接受治疗后1疗程、2疗程、3疗程及治疗结束,观察组溃疡面积均低于对照组(P<0.05)。结论在糖尿病足临床治疗方面,益气通络汤联合推拿辅助治疗疗效显著,可有效调节血糖,减轻炎症反应,促进创面恢复,值得临床应用推广。

Objective To explore the effect of Yiqi Tongluo Decoction combined with massage on the wound recovery in diabetic foot patients.Methods A total of 96 patients with diabetic foot ulcers were randomly divided into two groups,with 48 cases in each group.Both groups were given a basic medical treatment.In addition,the control group was treated with conventional drugs and a wound treatment,and the observation group was treated with Yiqi Tongluo Decoction combined with massage on the basis of the control group.The clinical effects,levels of the blood glucose metabolic indicators of glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG),2h postprandial blood glucose(2h FPG),levels of the serum inflammatory factor C-reactive protein(CRP)and cell necrosis factor alpha(TNF-α)and the wound recovery of the two groups were compared.Results The total effective rate in the observation group was higher than that in the control group(93.75%vs 79.17%)(P<0.05).After treatment,the levels of HbA1c,FPG and 2hFPG in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CRP and TNF-αin the observation group were lower than those in the control group(P<0.05).Before treatment,there was no significant difference in ulcer area between the two groups(P>0.05),while the ulcer area of the observation group was lower than that of the control group after one course,two courses,three courses and at the end of treatment(P<0.05).Conclusion In the clinical treatment of diabetes foot,Yiqi Tongluo Decoction combined with massage has a significant therapeutic effect,which can effectively regulate the blood glucose,reduce inflammatory reactions and promote the wound recovery,worthy of clinical application and promotion.
