基于数据挖掘技术探讨针灸治疗寻常型银屑病选穴规律

Acupoint Selection Law of Acupuncture and Moxibustion in Treating Psoriasis Vulgaris Based on Data Mining Technology

目的探讨针灸治疗寻常型银屑病的临床选穴规律。方法检索中国生物医学文献数据库(CBM)、中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(维普网)、PubMed、Cochrane Libarary收录的2002年1月1日-2022年1月7日针灸治疗寻常型银屑病的临床研究,建立针灸处方数据库,进行描述性分析、聚类分析及关联规则分析,对选穴规律进行分析总结。结果筛选后纳入179篇文献,207首处方,共涉及腧穴75个,总频次为1118次,高频腧穴为曲池、阿是穴、足三里、肺俞、血海、膈俞等16个腧穴;选穴位置主要集中在背部与四肢;高频经脉为足太阳膀胱经、手阳明大肠经、督脉、足太阴脾经;特定穴共使用40个,总频次为678次(65.5%)。聚类分析得到4个有效聚类群:肾俞-肝俞-心俞-脾俞-肺俞-膈俞,合谷-委中,大椎-灵台,血海-三阴交-曲池-足三里-风市。关联规则分析得到以曲池、足三里、肺俞、血海、膈俞、大椎、肾俞、三阴交、肝俞、合谷、脾俞、心俞、委中为核心的处方。结论寻常型银屑病选穴集中在背部与四肢,以阳经经穴为主,重视特定穴,选穴与治法紧密相关,针对病机,调和气血,标本兼顾。

Objective To investigate the acupoint selection law in clinic of acupuncture and moxibustion in the treatment of psoriasis vulgaris.Methods Clinical study of acupuncture and moxibustion in the treatment of psoriasis vulgaris was retrieved from CBM,CNKI,Wanfang Data,VIP,PubMed and Cochrane Library from 1st Jan 2002 to 7th Jan 2022.The database of acupuncture and moxibustion prescriptions was established.Descriptive analysis,cluster analysis and association rule analysis were carried out,and the acupoint selection law was analyzed and summarized.Results A total of 179 articles and 207 prescriptions were included,involving 75 acupoints,with a total frequency of 1118 times.16 high-frequency acupoints were obtained,such as Quchi(LI11),Ashi,Zusanli(ST36),Feishu(BL13),Xuehai(SP10),and Geshu(BL17).Acupoint selection was mainly focused on the back and limbs;the high-frequency meridians were the bladder meridian of foot-taiyang,large intestine meridian of hand-yangming,governor vessel and spleen meridian of foot-taiyin;a total of 40 specific acupoints were used,with a total frequency of 678 times,accounting for 65.5%of the total frequency.Four effective clusters were obtained by cluster analysis:Shenshu(BL23)-Ganshu(BL18)-Xinshu(BL15)-Pishu(BL20)-Feishu(BL13)-Geshu(BL17),Hegu(LI4)-Weizhong(BL40),Dazhui(GV14)-Lingtai(GV10),Xuehai(SP10)-Sanyinjiao(SP6)-Quchi(LI11)-Zusanli(ST36)-Fengshi(GB31).The association rules analysis obtained the prescriptions with Quchi(LI11),Zusanli(ST36),Feishu(BL13),Xuehai(SP10),Geshu(BL17),Dazhui(CV14),Shenshu(BL23),Sanyinjiao(SP6),Ganshu(BL18),Hegu(LI4),Pishu(BL20),Xinshu(BL15)and Weizhong(BL40)as the core.Conclusion The acupoint selection for the treatment of psoriasis vulgaris focus on the back and limbs,yang meridians and special acupoints.The selection of acupoints is closely related to the treatment method,aiming at the pathogenesis,regulating qi and blood,and taking into account the manifestation and root cause of disease.
