人际关系理论在慢性乙型肝炎肝硬化患者护理改进中的应用研究

Application of Interpersonal Relationship Theory in Nursing Improvement of Patients with Chronic Hepatitis B Cirrhosis

目的:探讨基于人际关系理论对慢性乙型肝炎肝硬化患者进行护理改进的策略,并分析其应用效果。方法:将2018年1月—2021年12月蚌埠医学院第二附属医院收治的88例慢性乙型肝炎肝硬化患者依照随机数表法分为常规组与改进组,各44例。常规组实施常规护理,改进组对护理模式采用人际关系理论进行改进。对比两组护理前后负性情绪和深层认知结构变化,诊疗配合度,护理前后肝功能变化和并发症。结果:护理后,两组焦虑、抑郁和深层认知结构评分及血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)水平较护理前均降低,且改进组以上指标均低于常规组(P<0.05);护理后,两组血清白蛋白(ALB)水平较护理前均升高,且改进组以上指标高于常规组(P<0.05)。改进组完全配合率高于常规组,并发症发生率低于常规组(P<0.05)。结论:在慢性乙型肝炎肝硬化患者护理中,采用人际关系理论进行改进效果良好。

Objective: To explore the strategy of nursing improvement for patients with chronic hepatitis B cirrhosis based on interpersonal relationship theory and analyze its application effect. Method: A total of 88 patients with chronic hepatitis B cirrhosis admitted to the Second Affiliated Hospital of Bengbu Medical College from January 2018 to December 2021 were randomly divided into the routine group and the improved group, 44 cases in each group. The routine group was given routine nursing, and the improved group was improved by interpersonal relationship theory. The changes of negative emotions and deep cognitive structure, diagnosis and treatment coordination, liver function changes before and after nursing and complications were compared between two groups. Result: After nursing, the scores of anxiety,depression, deep cognitive structure, serum alanine aminotransferase(ALT), aspartate aminotransferase(AST) and total bilirubin(TBIL) in two groups were decreased compared with those before nursing, and the above indexes in the improved group were lower than those in the routine group(P<0.05). After nursing, the serum albumin(ALB) levels of two groups were higher than those before nursing, and the above index of the improved group was higher than that of the routine group(P<0.05). The complete cooperation rate of the improved group was higher than that of the routine group, and the incidence of complications was lower than that in the routine group(P<0.05). Conclusion: In the nursing of patients with chronic hepatitis B and cirrhosis, the improvement of interpersonal relationship theory is effective.
