称重饮食在新诊断2型糖尿病患者胰岛素泵强化治疗中的应用

Application of Weighing Diet in Intensive Treatment of Insulin Pump of Newly Diagnosed Type 2 Diabetes Mellitus Patients

目的:观察称重饮食在新诊断2型糖尿病(T2DM)患者胰岛素泵强化治疗中的应用。方法:选取2018年10月—2020年11月深圳市宝安人民医院(集团)收治的92例新诊断T2DM患者为研究对象。根据随机数表法将其分为观察组和对照组,各46例。两组均给予胰岛素泵强化治疗,对照组给予常规护理,观察组在对照组基础上给予称重饮食。比较两组护理前后血糖指标、胰岛功能,低血糖发生率及护理满意度。结果:护理后,观察组空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)水平均低于对照组(P<0.05)。护理后,观察组胰岛素抵抗指数(HOMA-IR)低于对照组,胰岛素分泌指数(HOMA-β)高于对照组(P<0.05)。观察组治疗期间低血糖发生率低于对照组(χ^(2)=4.929,P<0.05)。观察组护理总满意度高于对照组(P<0.05)。结论:新诊断T2DM患者胰岛素泵强化治疗中应用称重饮食,能够较好地改善血糖水平、胰岛功能,在早期起到一定的逆转效果,低血糖发生率较低,护理满意度高。

Objective: To observe the application of weighing diet in intensive treatment of insulin pump of patients with newly diagnosed type 2 diabetes mellitus(T2DM). Method: A total of 92 newly diagnosed T2DM patients admitted to Shenzhen Bao’an People’s Hospital(Group) from October 2018 to November 2020 were selected as the study subjects. They were divided into observation group and control group according to the random number table method, 46 cases in each group. Both groups were given intensive treatment of insulin pump, the control group was given routine nursing, and the observation group was given weighing diet on the basis of the control group.The blood glucose index, islet function before and after nursing, hypoglycemia incidence and nursing satisfaction were compared between the two groups. Result: After nursing, the levels of fasting blood glucose(FPG), 2 h postprandial blood glucose(2 h PG), glycosylated hemoglobin(HbA1c) in the observation group were lower than those in the control group(P<0.05). After nursing, the insulin resistance index(HOMA-IR) of the observation group was lower than that of the control group, and the insulin secretion index(HOMA-β) was higher than that of the control group(P<0.05). The incidence of hypoglycemia in the observation group was lower than that in the control group during treatment( χ^(2)=4.929, P<0.05). The total nursing satisfaction of the observation group was higher than of the control group(P<0.05). Conclusion: The application of weighing diet in the intensive treatment of insulin pump of newly diagnosed T2DM patients can better improve blood glucose level and islet function, and play a certain reversal effect in the early stage. The incidence of hypoglycemia is low, and the nursing satisfaction is high.
