千金苇茎汤加减联合肺泡灌洗治疗重症肺炎的临床研究

Clinical Study on the Treatment of Severe Pneumonia with Qianjin Weijing Decoction Add and Subtract Combined with Alveolar Lavage

目的:探究在重症肺炎患者中实施千金苇茎汤加减联合肺泡灌洗治疗的临床效果。方法:选取2019年6月—2022年6月于常熟市中医院(新区医院)重症医学科治疗的重症肺炎患者100例作为研究对象,通过随机数表法将其分为对照组(n=50)和观察组(n=50)。对照组给予西医治疗,观察组在对照组的基础上给予千金苇茎汤加减联合肺泡灌洗治疗。对比两组患者的临床疗效,治疗前后的中医症候积分、肺功能指标及白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)。结果:观察组的临床有效率高于对照组(P<0.05);观察组治疗2周后的中医症候积分低于对照组(P<0.05);观察组治疗2周后的用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC均高于对照组(P<0.05);观察组治疗2周后的IL-6、CRP及TNF-α水平均低于对照组(P<0.05)。结论:在重症肺炎患者中实施千金苇茎汤加减联合肺泡灌洗治疗可提高患者的临床效果,改善其临床症状、肺功能并降低其炎症水平。

Objective: To explore the clinical effect of Qianjin Weijing Decoction add and subtract combined with alveolar lavage in patients with severe pneumonia. Method: A total of 100 patients with severe pneumonia treated in the Intensive Care Department of Changshu Traditional Chinese Medicine Hospital(New District Hospital) from June 2019 to June 2022 were selected as the research objects, they were divided into control group(n=50) and observation group(n=50) by random number table method. The control group was treated with Western medicine, and the observation group was treated with Qianjin Weijing Decoction add and subtract combined with alveolar lavage on the basis of control group. The clinical efficacy, TCM symptom score, lung function index, interleukin-6(IL-6), C-reactive protein(CRP), tumor necrosis factor-α(TNF-α) before and after treatment were compared between the two groups. Result: The clinical effective rate of observation group was higher than that of control group(P<0.05). After 2 weeks of treatment, the TCM syndrome score of the observation group was lower than that of the control group(P<0.05). The forced vital capacity(FVC), forced expiratory volume at the first second(FEV_(1)) and FEV_(1)/FVC in the observation group after 2 weeks of treatment were higher than those in the control group(P<0.05).After 2 weeks of treatment, the levels of IL-6, CRP and TNF-α in observation group were lower than those in control group(P<0.05).Conclusion: In patients with severe pneumonia, Qianjin Weijing Decoction add and subtract combined with alveolar lavage can improve the clinical effect, improve the clinical symptoms, lung function and reduce the level of inflammation.
