摘要
目的:研究冠心病患者冠脉病变程度与血清基质金属蛋白酶2(MMP2)、可溶性CD40配体(sCD40L)、Ⅰ型胶原羧基末端肽(ⅠCTP)、胱抑素C(Cys C)水平的关系。方法:回顾性选取2020年10月—2021年10月监利市中医医院50例冠心病患者作为冠心病组,另选取同期本院健康体检人员50例作为健康组。检测两组血清指标,评估冠心病组患者冠脉狭窄程度。比较两组MMP2、sCD40L、ⅠCTP、Cys C水平。比较冠心病组不同疾病类型、病变支数、冠脉狭窄程度患者MMP2、sCD40L、ⅠCTP、Cys C水平。分析冠心病患者Gensini评分与MMP2、sCD40L、ⅠCTP、Cys C水平的相关性。结果:冠心病组血清MMP2、ⅠCTP、Cys C、sCD40L水平均显著高于健康组(P<0.05)。稳定型心绞痛、不稳定型心绞痛、急性心肌梗死患者的血清MMP2、ⅠCTP、Cys C、sCD40L水平均逐渐升高(P<0.05)。单支病变、双支病变、三支病变患者的血清MMP2、ⅠCTP、Cys C、sCD40L水平均逐渐升高(P<0.05)。冠心病组中轻度狭窄14例,中度狭窄16例,重度狭窄20例,占比分别为28.00%、32.00%、40.00%。轻度狭窄、中度狭窄、重度狭窄患者的血清MMP2、ⅠCTP、Cys C、sCD40L水平均逐渐升高(P<0.05)。冠心病患者Gensini评分与血清MMP2、ⅠCTP、Cys C、sCD40L水平均呈显著的正相关关系(P<0.05)。结论:冠心病患者冠脉病变程度与血清MMP2、ⅠCTP、Cys C、sCD40L水平呈正相关关系。
Objective: To study the relationship between the degree of coronary artery disease and levels of serum matrix metalloproteinase 2(MMP2), soluble CD40 ligand(sCD40L), typeⅠ collagen carboxy terminal peptide(ⅠCTP) and cystatin C(Cys C)in patients with coronary heart disease. Method: From October 2020 to October 2021, 50 patients with coronary heart disease in Jianli Hospital of Traditional Chinese Medicine were retrospectively selected as the coronary heart disease group, and 50 healthy physical examination personnel in our hospital at the same period were selected as the healthy group. The serum indexes of the two groups were detected, and the degree of coronary artery stenosis of patients in coronary heart disease group was evaluated. The levels of MMP2,sCD40L, ⅠCTP and Cys C were compared between the two groups. The levels of MMP2, sCD40L, ⅠCTP and Cys C of patients with different types of disease, number of diseased branches and degree of coronary artery stenosis in coronary heart disease group were compared. The correlation among Gensini score and MMP2, sCD40L, ⅠCTP and Cys C levels in patients with coronary heart disease was analyzed. Result: The levels of serum MMP2, ⅠCTP, Cys C and sCD40L in coronary heart disease group were significantly higher than those in healthy group(P<0.05). The levels of serum MMP2, ⅠCTP, Cys C and sCD40L were gradually increased in patients with stable angina, unstable angina and acute myocardial infarction(P<0.05). The levels of serum MMP2, ⅠCTP, Cys C and sCD40L in patients with single-vessel disease, double-vessel disease and three-vessel disease were gradually increased(P<0.05). In the coronary heart disease group, there were 14 cases with mild stenosis, 16 cases with moderate stenosis and 20 cases with severe stenosis, accounting for 28.00%, 32.00% and 40.00% respectively. The levels of serum MMP2, ⅠCTP, Cys C and sCD40L were gradually increased in patients with mild stenosis, moderate stenosis and severe stenosis(P<0.05). Gensini score was positively correlated with serum MMP2, ⅠCTP, Cys C and sCD40L levels in patients with coronary heart disease(P<0.05). Conclusion: The degree of coronary artery disease was positively correlated with serum levels of MMP2, ⅠCTP, Cys C and sCD40L in patients with coronary heart disease.
