鼻内镜下等温等离子腺样体切除术治疗分泌性中耳炎并腺样体肥大患儿的临床效果

Clinical Effect of Isothermal Plasma Adenoidectomy under Nasal Endoscope in the Treatment of Secretory Otitis Media with Adenoid Hypertrophy

目的:探析应用鼻内镜下等温等离子腺样体切除术对分泌性中耳炎并腺样体肥大患儿的临床症状改善情况、听力水平及炎症因子的影响。方法:选取2021年8月—2022年6月102例在武汉济和医院进行手术治疗的分泌性中耳炎并腺样体肥大患儿为研究对象,根据不同的手术方式分为两组,其中参照组(n=51)给予常规鼻内镜下切吸钻腺样体切除术,研究组(n=51)则实施鼻内镜下等温等离子腺样体切除术,对比两组临床效果。结果:研究组临床症状发生率低于参照组,差异有统计学意义(P<0.05)。研究组不同频率的听力阈值低于参照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为1.96%,低于参照组的23.52%(P<0.05)。治疗后,两组免疫球蛋白E(IgE)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)降低,且研究组低于参照组,差异有统计学意义(P<0.05)。结论:针对分泌性中耳炎并腺样体肥大患儿,鼻内镜下等温等离子腺样体切除术取得显著成效,不仅提高手术成功率,明显改善鼻塞、耳鸣耳痛等临床症状,同时提高听力水平,进一步降低并发症发生率。

Objective:To explore the effect of isothermal plasma adenoidectomy under nasal endoscope on the improvement of clinical symptoms,hearing level and inflammatory factors in children with secretory otitis media complicated with adenoid hypertrophy.Method:From August 2021 to June 2022,102 children with secretory otitis media and adenoid hypertrophy who underwent surgery in Wuhan Jihe Hospital were selected as the study subjects.They were divided into two groups according to different surgical methods.The reference group(n=51)was given the normal nasal endoscopic cutting aspiration drilling adenoidectomy,and the study group(n=51)was given the isothermal plasma adenoidectomy under nasal endoscope.The clinical effects of the two groups were compared.Result:The incidence of clinical symptoms in the study group were lower than those in the reference group,and the differences were statistically significant(P<0.05).The hearing threshold of different frequencies in the study group were lower than those in the reference group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the study group was 1.96%,which was lower than 23.52%in the reference group(P<0.05).After treatment,immunoglobulin E(IgE),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interleukin-2(IL-2)were decreased in the two groups,and the study group were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion:For children with secretory otitis media complicated with adenoid hypertrophy,isothermal plasma adenoidectomy under nasal endoscope has achieved remarkable results,which not only improves the success rate of surgery,significantly improves nasal congestion,tinnitus and algia and other clinical symptoms,but also improves hearing level and further reduces the incidence of complications.
