2型糖尿病患者早期肾损伤的影响因素及尿L-FABP、β_(2)-MG的诊断价值

Influencing Factors of Early Renal Injury in Patients with Type 2 Diabetes and the Diagnostic Value of Urinary L-FABP and β_(2)-MG

摘要 目的:研究2型糖尿病患者早期肾损伤的影响因素及尿肝型脂肪酸结合蛋白(L-FABP)、β_(2)-微球蛋白(β_(2)-MG)的诊断价值。方法:选取2019年1月—2021年12月苏州市相城人民医院104例2型糖尿病患者作为观察组,同时选取同期健康体检者105例作为对照组。观察组给予规范化治疗,同时检查所有研究对象尿L-FABP、β_(2)-MG水平,收集观察组患者一般资料,并根据观察组患者是否发生早期肾损伤分为肾损伤组与非肾损伤组。比较观察组和对照组尿L-FABP、β_(2)-MG水平。对2型糖尿病患者早期肾损伤进行单因素及多因素分析。分析尿L-FABP、β_(2)-MG对2型糖尿病患者早期肾损伤的诊断价值。结果:观察组尿L-FABP、β_(2)-MG水平均高于对照组(P<0.05)。104例2型糖尿病患者38例发生早期肾损伤,66例没有发生早期肾损伤。肾损伤组尿L-FABP及β_(2)-MG水平均高于非肾损伤组(P<0.05)。经logistic回归分析,尿L-FABP、β_(2)-MG为早期肾损伤的危险因素(P<0.05)。经ROC分析显示,尿L-FABP、β_(2)-MG单一诊断2期糖尿病患者早期肾损伤的AUC值为0.742、0.826,联合诊断的AUC值为0.874。结论:2型糖尿病早期肾损伤患者尿L-FABP、β_(2)-MG水平明显异常升高,且是早期肾损伤的危险因素,两者联合对早期肾损伤具有较高诊断价值。 Objective:To study the influencing factors of early renal injury in patients with type 2 diabetes and the diagnostic value of urinary liver type fatty acid binding protein(L-FABP)andβ_(2)-microglobulin(β_(2)-MG).Method:A total of 104 patients with type 2 diabetes in Suzhou Xiangcheng People's Hospital from January 2019 to December 2021 were selected as the observation group,and 105 healthy people who underwent physical examination at the same period were selected as the control group.The observation group was given standardized treatment,and the urinary L-FABP andβ_(2)-MG levels of all study subjects were examined.The general data of the observation group was collected,and the patients in the observation group were divided into renal injury group and non-renal injury group according to whether early renal injury occurred.Urinary L-FABP andβ_(2)-MG levels were compared between the observation group and the control group.Univariate and multivariate analysis of early renal injury in patients with type 2 diabetes mellitus were performed.The diagnostic value of urinary L-FABP andβ_(2)-MG for early renal injury in patients with type 2 diabetes mellitus was analyzed.Result:The levels of urinary L-FABP andβ_(2)-MG in the observation group were higher than those in the control group(P<0.05).Among 104 patients with type 2 diabetes,38 cases had early renal injury,and 66 cases had no early renal injury.The levels of urinary L-FABP andβ_(2)-MG in the renal injury group were higher than those in the non-renal injury group(P<0.05).logistic regression analysis showed that urine L-FABP andβ_(2)-MG were risk factors for early renal injury.ROC analysis showed that the AUC values of urinary L-FABP andβ_(2)-MG for single diagnosis of early renal injury in patients with stage 2 diabetes were 0.742 and 0.826,the AUC value of combined diagnosis was 0.874.Conclusion:Urinary L-FABP andβ_(2)-MG levels are significantly abnormally increased in type 2 diabetic patients with early renal injury,which are risk factors for early renal injury.The combination of L-FABP andβ_(2)-MG has high diagnostic value for early renal injury.
