血清颗粒体蛋白与胃蛋白酶原、胃泌素-17在胃癌早期诊断中的应用价值 被引量:3

Study on the application value of serum granular protein,pepsinogen and gastrin-17 in early diagnosis of gastric cancer

摘要 目的 研究血清颗粒体蛋白(GRN)检测在胃癌早期筛查中的诊断价值和联合检测意义。方法 纳入本院2021年1月—12月收治的50例胃癌患者,50例胃良性病变患者和50例健康体检患者作为研究对象,空腹血液检测PGⅠ、PGⅡ、G-17和GRN水平,采用单因素、logistic回归、ROC曲线分析比较血清GRN与其他检测指标对胃癌筛查的诊断价值。结果 单因素分析显示,胃癌组PGⅠ和PGR随着胃病变程度增加而降低,PGⅡ、G-17和GRN随着胃病变程度严重而升高,五项指标在胃癌组与良性病变组、对照组比较差异均有统计学意义(P<0.05);logistic回归分析显示,PGⅠ、PGⅡ、G-17和GRN差异均有统计学意义(P<0.05),而PGR差异无统计学意义(P>0.05);ROC曲线分析显示联合检测曲线下面积最大,为0.925。结论 GRN在胃癌患者中呈现高表达,在胃癌早期筛查中具有良好的诊断价值,但是与PGⅠ、PGⅡ、PGR、G-17联合检测对胃癌的早期筛查效果更好。 Objective This paper aims to study the diagnostic value and the combined detection value of serum granular protein(GRN) in the early screening of gastric cancer. Methods Fifty patients with gastric cancer, fifty patients with benign gastric lesions and fifty healthy physical examination patients admitted to our hospital from January to December 2021 were enrolled as subjects. The levels of PGⅠ, PGⅡ, G-17 and GRN were detected by fasting blood sampling. Single factor, logistic regression and ROC curve analysis were used to compare the diagnostic values of serum GRN and other detection indexes in gastric cancer screening. Results Univariate analysis showed that PGⅠ and PGR decreased with the increase of gastric lesions, while PGⅡ, G-17 and GRN increased with the increase of gastric lesions. There was statistical significance on the differences of the above five index among gastric cancer group, benign lesion group and control group(P<0.05). Logistic regression analysis showed that there was statistical significance on the differences of PⅠ, PⅡ, G-17 and GRN(P<0.05), but not on the difference of PGR(P>0.05). ROC curve analysis showed that the area under the curve(AUC) of the combined detection was the largest(0.925). Conclusion GRN is highly expressed in patients with gastric cancer, which has good diagnostic value in early screening of gastric cancer. However, the combined detection of PGⅠ, PGⅡ, PGR and G-17 has better effect on the early screening of gastric cancer.
