四逆汤联合阿加曲班对缓解下肢动脉硬化闭塞患者症状及血清miR-342-5p和miR-93水平的影响

Effect of Sini Decoction combined with argatroban on symptom relief and serum miR-342-5p and miR-93 levels in patients with lower extremity arteriosclerosis occlusion

目的 探讨四逆汤联合阿加曲班对下肢动脉硬化闭塞患者症状的缓解及其可能机制。方法 选取本院在2020年6月—2021年6月收治的下肢动脉硬化闭塞患者104例,按照随机数字表法分为观察组和对照组,每组各52例。对照组采用阿加曲班进行治疗,观察组使用四逆汤联合阿加曲班进行治疗。检测所有患者治疗前后的临床指标、血清miR-342-5p、miR-93水平、血液流变学指标以及临床疗效。结果 治疗后,观察组患者肤温降低、间歇性跛行及静息痛评分、血清miR-342-5p和miR-93水平、全血黏度、血浆黏度、纤维蛋白原、血小板黏附率、红细胞聚集指数低于对照组,踝/肱动脉压比值和治疗总有效率高于对照组,差异均有统计学意义(P<0.05)。结论 四逆汤联合阿加曲班治疗下肢动脉硬化闭塞,能够缓解患者临床症状,改善血液流变学指标,可能与降低血清miR-342-5p、miR-93水平有关。

Objective This paper aims to investigate the effect of Sini Decoction combined with argatroban on the relief of symptoms and its mechanism. Methods A total of 104 patients with lower extremity arteriosclerosis and occlusion treated in our hospital from June 2020 to June 2021 were randomly divided into observation group and control group, with 52 cases in each group. The control group was treated with argatroban, and the observation group was treated with Sini Decoction on the basis of the control group. The clinical indicators, serum levels of miR-342-5p and miR-93, hemorheology indicators and clinical efficacy were compared before and after treatment between the two groups. Results After treatment, the skin temperature decreased, intermittent claudication and resting pain scores, serum miR-342-5p and miR-93 levels, whole blood viscosity, plasma viscosity, fibrinogen, platelet adhesion rate, and erythrocyte aggregation index in the control group were lower than that in the observation group, the ratio of ankle/brachial artery pressure and the total effective rate were higher than those in the control group, with the above differences statistically significant(P<0.05). Conclusion Sini Decoction combined with argatroban in the treatment of lower extremity arteriosclerosis and occlusion can relieve clinical symptoms and improve hemorheological indexes, which may be related to the reduction of serum miR-342-5p and miR-93 levels.
