卒中患者呼吸道多重耐药感染发病率及影响因素的logistic回归分析 被引量:1

Logistic regression analysis of incidence rate and influencing factors of respiratory multidrug resistance infection in stroke patients

摘要 目的 分析卒中患者呼吸道多重耐药(MDR)感染发病率,并探讨其影响因素。方法 回顾性采集杭州市临安区第一人民医院2021年1—12月200例卒中患者临床资料,于患者住院期间进行细菌培养及药敏试验,评估患者呼吸道MDR感染情况;查阅患者病历资料,记录患者基线资料及实验室指标[白蛋白(ALB)],采用logistic回归分析影响卒中患者呼吸道MDR感染的相关因子。结果 200例卒中患者中23例次发生呼吸道MDR感染,感染发生率为11.50%;MDR感染组与非MDR感染组卒中类型、吞咽障碍情况、血清ALB水平比较,差异有统计学意义(P<0.05);2组其他资料比较,差异无统计学意义(P>0.05);logistic回归分析结果显示,出血性卒中、有吞咽障碍、血清ALB低表达均是卒中患者呼吸道MDR感染的影响因素(P<0.05)。结论 卒中患者呼吸道MDR发生较高,可能受出血性卒中、伴吞咽障碍、血清ALB低表达等因素的影响,临床需重视上述因素,以此降低呼吸道MDR发生率。 Objective This paper aims to analyze the incidence rate of respiratory multidrug resistance(MDR) infection in stroke patients and explore its influencing factors. Methods The clinical data of 200 stroke patients in Lin’an District First People’s Hospital of Hangzhou City from January 2021 to December 2021 were collected retrospectively. Bacterial culture and drug sensitivity test were carried out during hospitalization to evaluate the respiratory MDR infection of patients;the patients’ medical records were consulted, and the patients’ baseline data and laboratory indicators [albumin(ALB)] were recorded. Logistic regression equation was used to analyze the related factors affecting respiratory MDR infection in stroke patients. Results Among the 200 stroke patients, 23 had respiratory MDR infection, and the incidence of infection was 11.50%;the differences of stroke type, dysphagia and serum ALB level between MDR infection group and non MDR infection group were statistically significant(P<0.05);there was no statistical significance on the difference in other data between the two groups(P>0.05);the results of logistic regression equation analysis showed that hemorrhagic stroke, dysphagia and low expression of serum ALB were the influencing factors of respiratory MDR infection in stroke patients(P<0.05). Conclusion The incidence of MDR in respiratory tract of stroke patients is high, which may be affected by hemorrhagic stroke, dysphagia, low expression of serum ALB and other factors. Clinical attention should be paid to the above factors to reduce the incidence of MDR in respiratory tract.
