經口支撐喉鏡下聲門區病變CO_(2)激光切除27例體會

Clinical experience of 27 glottic diseases resected by transoral carbon dioxide laser

目的探討CO_(2)激光經口支撐喉鏡下治療聲門區病變手術的臨床應用。方法收集2015年1月~2015年8月本院收治的聲門區病變患者共27例,其中,良性病變26例,包括聲帶息肉21例、聲帶囊腫2例、雙側聲帶白斑1例、雙側聲帶任克氏間隙水腫1例、多發性喉乳頭狀瘤1例;惡性病變1例,為聲門型喉癌。全部病例均應用CO_(2)激光經口支撐喉鏡下顯微鏡下切除病變組織。結果所有病變均完整切除。術後隨訪,術腔上皮化良好,未見併發症及復發。結論經口支撐喉鏡下聲門區病變CO_(2)激光切除效果好,尤其對於聲門型喉癌患者治療創傷性小,療效佳,有利於喉完整性保留及嗓音恢復。

Objective To investigate the clinical applications of transoral carbon dioxide laser resection for glottic diseases.Methods 27 cases receiving transoral CO_(2)laser glottic neoplasms resection in Kiang Wu hospital from January to August 2015 were reviewed,including 26 benign diseases(21 vocal cord polyp,2 vocal cord cyst,1 bilateral vocal cord leukoplakia,1 bilateral Reinke edema,1 multiple papilloma)and 1 malignant tumor.Results All lesions were completely resected by CO_(2)laser.Surgical area was well epithelialized without obvious complication or relapse.Conclusion Transoral CO_(2)laser resection is associated with good treatment outcome,especially for glottic carcinoma.The treatment side effect is minimal,with better laryngeal function reserve and voice recovery.
