左旋布比卡因和布比卡因用於剖宮產腰麻的應用比較

目的比較0.5%左旋布比卡因和0.5%布比卡因用於剖宮產術腰麻的臨床應用效果。方法腰麻下擇期剖宮產術的產婦39例,隨機分為左旋布比卡因組和布比卡因組,比較兩組產婦的麻醉效果、術中麻黃素的使用量、產婦下肢運動評分(Bromage scales)及新生兒1min及5min APGAR評分。結果兩組產婦均取得滿意的麻醉效果(P>0.05);左旋布比卡因組產婦麻黃素用量較布比卡因組產婦少,其差異有統計學意義(P<0.05);左旋布比卡因組產婦下肢運動評分較布比卡因組低,其差異有統計學意義(P<0.05);兩組新生兒APGAR評分差異無統計學意義(P>0.05)。結論左旋布比卡因用於剖宮產術腰麻效果滿意,血流動力學更穩定,產婦術後恢復快,對新生兒無不良影響,臨床上使用安全可靠。

Objective To compare the anesthetic effect of 0.5%isobaric levobupivacaine versus bupivacaine in cesarean section.Methods 39 patient undergoing caesarean section under spinal anesthesia were randomly assigned to receive injections of 0.5%isobaric levobupivacaine 3ml or bupivacaine 3ml.The effect of the anesthetics,usage of ephedrine,Bromage scores of legs,1 min-and 5 min-Apgar's scores for neonates were noted.Results There were no significances between the two groups in the effect of the anesthetics(P>0.05).There were less usage of ephedrine in the levobupivacaine group than the bupivacaine group(P<0.05).Bromage scores of legs were lower in the levobupivacaine group than the bupivacaine group(P<0.05).There were no significances between the two groups in the Apgar's scores(P>0.05).Conclusion Spinal anesthesia with 0.5%isobaric levobupivacaine in caesarean section is safe and effective and achieves an ideal anesthetic effect,and there is no adverse effects in neonates.
