超聲心動圖診斷心肌緻密化不全5例報導

5 cases of noncompaction of ventricular myocardium diagnosed by echocardiography

目的探討超聲心動圖對心肌緻密化不全(NVM)的診斷價值。方法採用超聲心動圖常規測量5例NVM各腔室內徑及心功能主要參數,分析二維、M型及彩色多普勒等特點及其臨床資料。結果5例患者的超聲心動圖均可見心室內出現大量粗大的肌小梁及其間深陷的隱窩,彩色多普勒可探及間隙內有血流與心室相通。4例有不同程度的左室擴大、心功能降低及瓣膜返流。結論NVM具有特徵性超聲改變,超聲心動圖是無創、敏感診斷心肌緻密化不全的首選確診方法。

Objective To investigate the value of echocardiography in diagnosis of noncompaction of ventricular myocardium(NVM).Methods The suspicious NVM patients were studied by echocardiography of two-dimension,M-mode and color doppler.The clinical index,cardiac structures and blood flow changes of NVM were obtained to observe and analyze.Results 2D showed the lesions appearing like myocardial“honeycomb”in 5 patients.Color doppler showed low blood flow signal in the gaps and communicated with the left ventricule.4 cases of NVM have different degree of left ventricular systolic function decrease,left ventricle dilatation and valve regurgitation.Conclusion NVM has the characteristic ultrasonic changes and the echocardiography can be used as the first-line examination method for the diagnosis of NVM.
