前列腺素類制劑與宮頸球囊促宮頸成熟的效果比較

Comparison of the prostaglandin E2-and cervical dilatation balloon-promoting effects on cervical maturation and labor

目的比較陰道用前列腺素類制劑(前列腺素E2栓劑,PGE2)與COOK宮頸擴張球囊促宮頸成熟及引產的效果。方法選擇2016年10月~2018年4月於本院引產的104例孕足月,但宮頸條件不成熟產婦為研究對象。採用隨機方式分為實驗組1和實驗組2,每組各52例。實驗組1採用可控釋地諾前列酮栓進行引產,實驗組2採用COOK宮頸擴張球囊引產。比較兩組產婦促宮頸成熟效果、引產結果及相關併發症情況等。結果實驗組2引產處理後Bishop評分、促宮頸成熟總有效率均顯著高於實驗組1;實驗組2誘發臨床時間顯著短於實驗組1,陰道分娩率顯著高於實驗組1;兩組產後2小時出血量、新生兒窒息情況和不良反應率相似。結論與藥物前列腺素E2足月妊娠引產相比較,應用宮頸擴張球囊,能更有效促宮頸成熟,縮短引產時間和提高陰道分娩率,且不增加產婦和新生兒風險,是一種理想的引產方法。

Objective To compare the effect of prostaglandin E2(PGE2)and cervical dilatation balloon(COOK double balloons)in promoting cervical maturation and inducing labor.Methods 104 full-term pregnant women with immature cervical conditions(cervical Bishop score<6 points)who had induced labor in our hospital from October 2016 to April 2018 were selected.Patients were equallydivided into two groups(test group 1 and test group 2).The patients in test group 1 were induced by conventional prostaglandin E2,while the patients in test group 2 were induced by COOK balloon to promote cervical maturation.The effects of promoting cervical ripening,results of induced labor and related complications were compared.Results The Bishop score and the total effective rate of promoting cervical ripening in the test group 2 was significantly higher than those in the test group 1.The clinical induction time in the test group 2 was significantly shorter than that in the test group 1,and the vaginal delivery rate was significantly higher than that in the test group 1.The postpartum hemorrhage,neonatal asphyxia and adverse reaction rate were similar between the 2 groups.Conclusion Compared with prostaglandin E2,cervical dilatation balloon might be an ideal method of inducing labor as it can promote cervical maturation more effectively,shorten the induction time and improve the vaginal delivery rate,whilstdoes not increase the maternal and neonatalrisks.
