成人食管異物89例臨床分析

Clinical analysis of 89 cases of adult with esophageal foreign body

目的總結成人食管異物的臨床特點,指導内鏡治療方式的選擇。方法回顧性分析成人食管異物患者的臨床資料,食管異物併發症的相關因素,胃鏡或食管鏡取異物成功率。結果食管異物患者共89例,男性28例,女性61例,平均年齡56.7±14.6歲,異物平均停留時間13.4±29.6小時。最常見部位食管上段(77.5%),常見異物為魚骨(61.8%)。經胃鏡取異物29例,成功取出異物28例(成功率96.6%);經食管鏡取異物60例,成功取出58例(成功率96.7%),兩種取異物方式成功率無統計學差異。所有患者中有13例發生併發症(13/89,14.6%),併發症相關因素分析提示異物大小和患者是否存在基礎病與併發症的發生有顯著性差異(P<0.05)。結論胃鏡或食管鏡取食管異物成功率都很高且各有優勢,食管異物併發症與異物大小及基礎病相關。

Objective To summarize the clinical characteristics of adult esophageal foreign bodies and guide the selection of endoscopic treatment.Methods Retrospective analysis and comparison of clinical data of adult patients with esophageal foreign bodies,related factors of esophageal foreign body complications,and success rate of gastroscopic or esophageal foreign body removal.Results There were 89 patients with esophageal foreign body,28 males and 61 females,with an average age of 56.7±14.6 years,and the average foreign body residence time was 13.4±29.6 hours.The most common part is the upper esophagus,and the common foreign body is fish bone.29 cases of foreign bodies were removed by gastroscope,and 28 cases were successfully removed(success rate 96.6%);60 cases of foreign bodies were removed by esophagoscope,and 58 cases were successfully removed(success rate 96.7%).There was no statistical difference in the success rate of the two methods of removing foreign bodies.There were 13 cases(14.6%)has complications.Esophageal foreign body complications are related to the size of the foreign body and the existence of underlying diseases.Conclusion Esophageal foreign body complications are related to the size of the foreign body and the underlying disease.Gastroscopy or esophagoscopy have their own advantages in removing esophageal foreign bodies,and there was no significant difference in the success rate.
