不同音乐辅助心肺复苏培训对学员远期按压效果和操作信心的影响 被引量:1

Influence of different music-assisted cardiopulmonary resuscitation training on the effect of compression and operation confidence of trainees

摘要 目的观察国内外不同音乐辅助心肺复苏(CPR)培训对学员培训当日及2个月后胸外按压效果及操作自信心的影响。方法选择北京积水潭医院近3年新入职的住院医师及护士135人进行培训,随机分为对照组、拉德斯基进行曲音乐组(简称外音组)、解放军进行曲音乐组(简称中音组)。对照组仅按指南要求进行培训,外音组培训时以拉德斯基进行曲(108 bpm)为背景音乐引导学员进行按压;中音组培训时以解放军进行曲(114 bpm)为背景音乐引导学员进行按压。三组培训及操作练习时间相同。比较三组培训后当日、2个月后平均按压频率、平均按压深度、按压频率达标率、按压深度达标率、按压分数;比较两个音乐组平均按压频率与音乐频率差值;考核后记录两个音乐组学员对音乐的知晓率、操作时可回忆率及操作自信心。结果三组培训当日及2个月后按压频率、按压分数、按压频率达标率差异有统计学意义(P<0.05)。中音组2个月后按压频率达标率与外音组比较有统计学差异(P<0.05)。中音组培训后当日、2个月后按压频率与音乐频率差均小于外音组(P<0.05)。外音组2个月后按压频率、按压分数、按压频率与音乐频率差与同组培训当日比较,差异有统计学意义(P<0.05)。中音组2个月后按压频率、按压分数、按压频率与音乐频率差与同组培训当日比较,差异无统计学意义(P>0.05)。中音组与外音组对音乐知晓率、2个月后操作时可回忆率、增强信心率差异有统计学意义(P<0.05)。结论在针对低年资医护的CPR模拟培训中,采用知晓率较高的中国音乐辅助胸外按压培训,有利于学员远期记忆形成,在整体按压质量及按压频率稳定性上可获得更优的效果,可增强学员完成CPR操作信心,适于在CPR培训中推广应用。 Objective To observe the influence of Chinese and foreign music-assisted cardiopulmonary resuscitation(CPR)training on the effect of chest compression and operation confidence of trainees on the day of training and two months later.Methods A total of 135 new resident doctors and nurses were selected and randomLy divided into the control group,music group of radetzky march(abbreviated as foreign music group)and music group of march of People’s Liberation Army(abbreviated as Chinese music group).The control group was only trained according to the guidelines.While the trainees in foreign music group were trained to conduct the compression with the Radetzky March(108 bpm)as the background music during the training;and the trainees in Chinese music group were trained to conduct the compression with March of People’s Liberation Army(114 bmp)as the background music.The training and operation practice time were the same in the three groups.The average compression frequency,average compression depth,compression frequency compliance rate,compression depth compliance rate and compression score of the three groups on the day of training and two months after training were compared;the difference value between average compression frequency and music frequency between the two music groups were compared;after the examination,the awareness rate of music,operation recall rate and operation confidence of the trainees in the two music groups were recorded.Results There were statistically significant differences(P<0.05)in the compression frequency,compression score and compression frequency compliance rate among the three groups on the day of training and two months later.There was statistically significant difference(P<0.05)in the compression frequency compliance rate between the Chinese music group and the foreign music group after 2 months.The difference of the compression frequency and music frequency of the Chinese music group on the day of training and two months after training was smaller than that of the foreign music group(P<0.05).There were statistically significant difference in compression frequency,compression score,compression frequency and music frequency of the foreign music group after two months from those of the same group on the day of training(P<0.05).There was no statistically significant difference of Chinese music group(P>0.05)in compression frequency,compression score,difference in compression frequency and music frequency between the day of training and two months later.There were statistically significant differences(P<0.05)in the awareness rate of music,the operation recall rate after two months,and the confidence enhancement rate between the Chinese music group and the foreign music group.Conclusion In the simulation training of cardiopulmonary resuscitation for junior doctors and nurses,the use of Chinese music-assisted chest compression training with high awareness rate is conducive to the formation of long-term memory of trainees,and can lead the trainees to achieve better results in the stability of overall compression quality and compression frequency,so as to enhance the operation confidence of trainees to complete cardiopulmonary resuscitation.Therefore,it is also suitable for popularization and application in cardiopulmonary resuscitation training.
