B超引导下置入留置针在置管困难老年患者增强CT检查中的应用效果

Application Effect of Ultrasound-Guided Indwelling Needle Placement in Enhanced CT Examination in Elderly Patients with Difficulty in Catheterization

目的观察B超引导下置入留置针在置管困难老年患者增强CT检查中的应用效果。方法选取2018年09月—2019年02月在苏州大学附属第一医院影像科行增强CT检查的患者为研究对象,其中2018年09月—11月42例符合纳入标准的患者作为对照组进行常规置管,2018年12月—2019年02月42例符合纳入标准的患者作为试验组行B超引导下置管,对两组患者一次置管成功率、操作时间、穿刺次数、穿刺耗材费用、对比剂外渗率及图片成像质量等进行比较。结果试验组一次置管成功率78.6%,高于对照组的45.2%,差异有统计学意义(P<0.05)。试验组操作时间(3.5±1.2)min,短于对照组的(10.2±4.7)min,试验组的穿刺耗材费用(74.6±10.5)元,低于对照组的(95.5±51.4)元,差异有统计学意义(P<0.05)。试验组图片成像质量优于对照组,差异有统计学意义(P<0.05)。结论B超引导下置入留置针在置管困难老年患者增强CT检查中具有优势,提高了诊疗效率和患者的就医体验。

Objective To investigate the application effect of ultrasound-guided indwelling needle placement in enhanced CT examination in elderly patients with difficulty in catheterization.Methods Patients who underwent enhanced CT examination in the Department of Imaging,The First Affiliated Hospital of Soochow University from September 2018 to February 2019 were selected as the study subjects.A total of 42 patients meeting the inclusion criteria from September to November 2018 were selected as the control group for routine catheterization,while 42 patients meeting the inclusion criteria from December 2018 to February 2019 were selected as the experimental group for ultrasound-guided catheterization.The success rate of one-time catheterization,operation time,puncture times,puncture consumables cost,extravasation rate of contrast agent and image quality were compared between the two groups.Results The success rate of one-time catheterization in experimental group was 78.6%,higher than that in control group(45.2%),and the difference between the two groups was statistically significant(P<0.05).The operation time of the experimental group was(3.5±1.2)min,which was shorter than that of the control group(10.2±4.7)min,respectively;the cost of puncture supplies of the experimental group was(74.6±10.5)yuan,which was lower than that of the control group(95.5±51.4)yuan;and the difference was statistically significant(P<0.05).The image quality of the experimental group was better than that of the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Ultrasound guided by placing needle in catheter difficult enhanced CT examination has advantages in elderly patients and improve the efficiency of diagnosis and treatment experience of patients.
