Corvis ST校正眼压与Pentacam系统校正眼压的对比研究

摘要 目的比较角膜生物力学分析仪(Corvis ST)测量的角膜生物力学校正眼压值(bIOP)与非接触眼压计(NCT)测量的眼压值通过Pentacam眼前节分析系统内置的5种眼压校正公式所得校正眼压值的差异。方法回顾性病例研究。选取2019年8月1日至9月30日在我院屈光手术中心行近视角膜屈光手术的患者76例(152只眼),分别应用Corvis ST、NCT进行术前及术后1个月的眼压测量,眼压分别纳入Corvis ST中的直接测量值IOP、bIOP,NCT,NCT联合Pentacam得出的5种校正眼压值,分别记录为Ehlers、Shah、Dresden、Orssengo/Pye和Kohlhaas校正眼压值。将NCT的5种校正眼压值与Corvis ST测量的bIOP进行对比研究。术前术后的眼压值比较采用配对t检验,bIOP分别与5种校正眼压值比较采用独立样本t检验。结果术前术后Ehlers、Dresden、Orssengo/Pye和Kohlhaas校正眼压值及术前术后bIOP、IOP、NCT 3种眼压测量值的差异具有统计学意义,仅Shah校正眼压值手术前后差异无统计学意义(t=-1.01,P=0.312)。术前术后bIOP的差别为(0.85±1.33)mmHg,IOP的差别为(3.15±1.61)mmHg。术前bIOP值均高于5种校正眼压值(P<0.001);术后bIOP与Ehlers校正眼压值无明显差异(P>0.05),与其他校正眼压值有明显差异(P<0.001)。结论bIOP更能反映眼压的真实情况,屈光术后Ehlers校正眼压值与bIOP一致性最好;Pentacam系统的眼压校正作用是对屈光术后准确测量眼压的有力补充,屈光术后Shah校正公式相对可靠。 Objective To compare the corrected intraocular pressure(IOP)values between corneal visualization scheimpflug technology(Corvis ST)and Pentacam.Methods A retrospective study was conducted on 152 eyes of 76 patients who underwent corneal refractive surgery in Dongguan Aier Eye Hospital from August 1st,2019 to September 30th,2019.IOP values were obtained with non-contact tonometer(NCT)and Corvis ST preoperatively and at 1 month postoperative,respectively.IOP from NCT was corrected by 5 IOP formulas of Pentacam system to generate corrected IOP,recorded as Ehlers corrected IOP,Shah corrected IOP,Dresden corrected IOP,Orssengo/Pye corrected IOP,and Kohlhaas corrected IOP,respectively.The difference between Pentacam corrected IOP and biomechanically corrected intraocular pressure(bIOP)from Corvis ST was analyzed.The paired t-test was applied to analyze the difference between preoperative and postoperative IOP values.The independent t-test was performed to analyze the difference of IOP values between two groups.Results There was significant difference between preoperative and postoperative IOP values as to Ehlers corrected IOP,Dresden corrected IOP,Orssengo/Pye corrected IOP,and Kohlhaas corrected IOP,bIOP,IOP and NCT,while there was no significant difference as to Shah corrected IOP(t=-1.01,P=0.312).The difference between pre-and post-operation stages was 0.85±1.33 mmHg as to bIOP,while IOP showed a higher mean of difference of 3.15±1.61 mmHg.Preoperatively,the bIOP value was significantly higher than each Pentacam corrected IOP(P<0.001).Postoperatively,there was no significant difference between Ehlers corrected IOP and bIOP(P>0.05),while there was significant difference between other Pentacam corrected IOP and bIOP(P<0.001).conclusions The bIOP could show the real IOP,and Ehlers corrected IOP was well consistent with bIOP postoperatively.Pentacam corrected system was an effective supplemental measure to obtain accurate IOP values after corneal refractive surgery,and Shah corrected IOP showed relative reliability after corneal refractive surgery.
