初产妇分娩镇痛中应用腰硬联合麻醉的效果观察

Observation on the Effect of Combined Spinal and Epidural Anesthesia in Labor Analgesia of Primipara

探究了初产妇分娩镇痛中应用腰硬联合麻醉的效果。选取聊城市妇幼保健院于2019年11月—2021年11月接诊的60例采取腰硬联合麻醉的初产妇作为研究对象,将其随机均分为观察组、对照组(各30例)。对照组患者选择常规的硬膜外麻醉方式,观察组患者选择腰硬联合麻醉的方式,比较两组患者的VAS评分、镇痛起效时间、产程时间、Apgar评分、不良反应及剖宫产率。数据显示,麻醉后,观察组VAS评分下降幅度小于对照组、患者镇痛的起效时间及产程时间均短于对照组(P<0.05)。同时,对照组的新生儿Apgar评分高于观察组(P<0.05);患者在麻醉后牵拉反应、恶心呕吐、呼吸困难及低血压等不良反应发生率高于观察组(P<0.05);剖宫产率也高于对照组(P<0.05)。由此可知,对分娩镇痛的初产妇选择腰硬联合麻醉的方式可明显提高治疗效果、缩短产程,也更有利于胎儿及产妇的安全。

To explore the effect of combined spinal epidural anesthesia in labor analgesia of primipara.60 primipara who were treated with combined spinal and epidural anesthesia in Liaocheng Women and Children Health Hospital from November 2019 to November 2021 were selected as research objects,and they were randomly divided into observation group and control group(30 cases each).The control group chose routine epidural anesthesia,while the observation group chose combined spinal and epidural anesthesia.The VAS score,onset time of analgesia,labor process time,Apgar score,adverse reactions and cesarean section rate of the two groups were compared.The data showed that after anesthesia,the decrease of VAS score in the observation group was less than that in the control group,and the onset time of analgesia and labor process time of patients were shorter than those in the control group(P<0.05).At the same time,Apgar score of newborns in the control group was higher than that in the observation group(P<0.05);After anesthesia,the side effects of traction reaction,nausea and vomiting,dyspnea and hypotension in patients were higher than those in the observation group(P<0.05);The cesarean section rate w as also higher than that of the control group(P<0.05).It can be seen that the combination of spinal and epidural anesthesia can significantly improve the therapeutic effect,shorten the labor process,and also be more conducive to the safety of the fetus and pregnant women.
