胃癌患者抑郁影响因素的范围综述

A review of the range of factors affecting depression in patients with gastric cancer

目的探讨胃癌患者发生抑郁的影响因素,为制定有效的预防、干预措施提供借鉴。方法检索CNKI、维普、万方、CBM、PubMed、Embase、Web of Science、The Cochrane Library、CINAHL、Scopus数据库,查找建库至2022年1月1日国内外公开发表的有关胃癌患者抑郁影响因素的研究,完成文献筛选、质量评价和资料提取,并报告研究结果。结果共纳入14篇文献,研究对象30305例,涉及危险因素29个。胃癌患者发生抑郁与社会人口学因素(性别、文化程度、职业、婚姻状况、经济状况、年龄)、疾病因素(病程、疾病复发时间、诊断时的肿瘤位置、疾病分期、基础疾病、躯体症状、疾病治疗方式、生存期)、社会心理因素(疾病恐惧、社会支持、自尊、担心医疗环境、疾病认知、Herth希望量表评分、心理弹性量表评分、忧伤型人格)以及其他因素(医学应对方式、体质量指数、中性粒细胞与淋巴细胞比值、出院时间、医疗支付方式、食欲、睡眠)有关。结论胃癌患者抑郁的发生与社会人口学、疾病、社会心理等多个因素有关。

Objective To summarize the influencing factors of depression in patients with gastric cancer and to provide reference for effective prevention and intervention measures.Methods The databases of CNKI,VIP,Wanfang,CBM,PubMed,Embase,Web of Science,The Cochrane Library,CINAHL and Scopus were searched to find the studies on the influencing factors of depression in patients with gastric cancer published at home and abroad from the establishment of the database to January 1,2022.The literature screening,quality evaluation and data extraction were completed,and the results were reported.Results A total of 14 articles were included,including 30,305 subjects,involving 29 risk factors.Depression in patients with gastric cancer is related to social demographic factors,disease factors,social psychological factors and other factors.Among them,socio-demographic factors include gender,education level,occupation,marital status,economic status,age;disease factors include disease course,time of disease recurrence,tumor location at diagnosis,disease stage,underlying disease,physical symptoms,disease treatment,and survival period;psychosocial factors include disease fear,social support,self-esteem,worry about medical environment,disease cognition,Herth hope scale score,psychological resilience scale score,type D personality;other factors include medical coping model,body mass index,neutrophil-to-lymphocyte ratio,time-since-discharge,medical payment method,appetite and sleep.Conclusion The occurrence of depression in patients with gastric cancer is related to social demography,disease,social psychology and other factors.
