宫腔镜下电切术联合曼月乐治疗子宫内膜息肉的疗效及术后复发相关分析

Efficacy of hysteroscopic electroresection combined with Mirena for endometrial polyps and postoperative recurrence

目的探讨应用宫腔镜下电切术联合曼月乐治疗子宫内膜息肉(EP)患者的疗效。方法选择漯河市中医院2020年6月—2021年6月收治的EP患者(n=82例)为本次探究对象,根据随机数字表法将入选患者分为观察组及对照组。对照组(n=41例)行单纯宫腔镜下电切术治疗,观察组(n=41例)应用宫腔镜下电切术+曼月乐治疗。观察两组患者的总有效率、并发症及复发情况。比较治疗前后患者的子宫内膜厚度、痛经程度评分(VAS)、月经量及性生活质量。结果观察组患者总有效率显著高于对照组,且与对照组相比,观察组的并发症发生风险、复发风险更低,数据相较有统计学意义(P<0.05)。两组患者治疗后子宫内膜厚度、月经量及痛经程度评分呈现明显降低趋势,性生活各项评分呈现明显增高趋势,差异具有统计学意义(P<0.05)。治疗前,子宫内膜厚度、月经量、VAS、性生活各项维度评分组间相较无统计学意义(P>0.05);治疗后,两组患者的子宫内膜厚度、月经量、VAS评分均呈现降低趋势;性欲、性高潮、性心理评分均呈现增高趋势,与对照组相比,观察组更优,差异具有统计学意义(P<0.05)。结论EP经宫腔镜下电切术+曼月乐治疗后,患者的月经量减少,子宫内膜变薄,经痛程度可缓解,并发症少,复发率低,性生活质量进一步提高。

Objective To explore the clinical effect and postoperative recurrence of hysteroscopic hysterotomy combined with Mirena in the treatment of endometrial polyps(EP).Methods The EP patients(n=82 cases)admitted to Luohe Hospital of Traditional Chinese Medicine from June 2020 to June 2021 were selected as the subjects of this exploration,and the selected patients were divided into observation group and control group according to the random number table method.The control group(n=41 cases)underwent hysteroscopic electrotherapy,and the observation group(n=41 cases)underwent hysteroscopic electrotherapy combined with Mirena treatment.The total effective rate complications and recurrence of both groups were observed.Compare the endometrial thickness,Visual analogue rating scale(VAS),menstrual volume,and sexual quality before and after treatment.Results The total effective rate of the patients in the observation group was significantly higher than that of the control group,and the risk of complications and recurrence was lower than that of the control group,which was statistically significant(P<0.05).In the two groups,the scores of endometrial thickness,menstrual volume and VAS showed a significant decrease,and the scores of sexual life showed a significant increased trend,and the difference was statistically significant(P<0.05).Before treatment,endometrial thickness,menstrual volume,VAS and sex dimensions were not statistically significant(P>0.05);After treatment,endometrial thickening,menstrual volume and VAS scores decreased;sexual desire,orgasm,compared with the control group,the difference was statistically significant(P<0.05).Conclusion After the treatment of endometrial polyps by hysteroscopic electroresection combined with Mirena,the patient’s menstrual volume decreased,the endometrial thickening becomed thinner,the degree of menstrual pain significantly relieved,fewer complications,the recurrence rate is low,and the quality of sexual life is further improved.
