温肾化痰祛瘀方联合社区综合干预对高血压早期肾损害的临床疗效观察

Clinical effect of Wenshen Huatan Quyu prescription combined with community comprehensive intervention on earlyrenaldamageofhypertension

目的观察温肾化痰祛瘀方联合社区综合干预对高血压早期肾损害的临床疗效。方法2019年1月-2021年6月选择友谊街道社区卫生服务中心进行登记的100例高血压早期肾损害患者,采用随机数字表法分为对照组、治疗组各50例。对照组采用西医常规药物降压治疗,并进行社区综合干预;治疗组在对照组基础上采用温肾化痰祛瘀方治疗,3个疗程(12周)后评价2组收缩压/舒张压(SBP/DBP)变化、实验室指标[血肌酐(SCr)、尿微量白蛋白(mAlb)、尿蛋白与肌酐比值(ACR)及肾小球滤过率(eGFR)]、临床疗效及安全性。结果治疗前,2组SBP/DBP以及SCr、mAlb、ACR、eGFR无明显差异(P>0.05);治疗后,治疗组SCr、mAlb、ACR低于对照组,eGFR高于对照组(P<0.05)。治疗组总有效率高于对照组(P<0.05),且2组随访期间均未出现明显不良反应事件。结论高血压早期肾损害患者在社区综合干预基础上采用温肾化痰祛瘀方治疗,可有效控制血压、保护肾功能,同时提高临床疗效。

Objective To observe the clinical effect of Wenshen Huatan Quyu prescription combined with community comprehensive intervention on early renal damage of hypertension.MethodsFrom January 2019 to June 2021,100 hypertensive patients with early renal impairment were enrolled in Youyi Street Community Health Service Center.The patients were divided into control group and treatment group by random number table,50 cases in each group.The control group received conventional western medicine antihypertensive treatment,and community comprehensive intervention;The treatment group was treated with Wen Shen Huatan Quyu prescription on the basis of the control group.The changes of systolic/diastolic blood pressure(SBP/DBP),laboratory indicators[serum creatinine(Scr),Micro albuminuria(mAlb),Albuminuria/creatinine ratio(ACR),Estimated glomerular filtration rate(eGFR)],clinical efficacy and safety were evaluated after 3 courses(12 weeks).Results Before treatment,there were no significant differences in SBP/DBP and laboratory indexes(Scr,mAlb,ACR,eCFR)between the two groups(P>0.05).After treatment,Scr,mAlb and ACR of the treatment group were lower than those of the control group,and eGFR of the treatment group was higher than that of the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05),and there were no obvious adverse events in the two groups during the follow-up.Conclusion On the basis of community comprehensive intervention,Wenshen Huatan Quyu prescription can effectively control blood pressure,protect renal function,and improve clinical efficacy in hypertensive patients with early renal damage.
