摘要
目的研究探讨交锁髓内钉早期动力化对下肢骨折患者患肢功能及骨密度的影响。方法选取2017年1月-2022年3月收治的80例下肢骨折患者为对象,随机分为对照组、观察组,各40例。对照组以交锁髓内钉治疗,观察组采用交锁髓内钉早期动力化治疗。对两组患者术后骨折愈合情况、患肢功能优良率、骨密度及骨再生因子、并发症发生率等观察指标进行统计比较。结果观察组后开始下床活动时间、住院时间、临床愈合时间、骨性愈合时间均小于对照组(P<0.05)。观察组患肢功能优良率为92.50%(37/40),高于对照组患者为72.50%(29/40)(P<0.05)。患者术后3个月时骨密度(BMD)、骨形成蛋白6(BMP6)、成纤维细胞生长因子-2(FGF-2)水平均高于对照组(P<0.05)。观察组并发症发生率为7.50%(3/40),低于对照组患者为25.00%(10/40)(P<0.05)。结论交锁髓内钉早期动力化对下肢骨折患者的治疗效果显著,能够促进患者骨折的愈合,缩短患者术后康复时间,提高患者患肢活动功能,提高患者的骨密度和骨再生因子,降低患者并发症发生率。
Objective To investigate the effect of early dynamic interlocking intramedullary nail on the function and bone mineral density of the affected limb in patients with lower limb fracture.Methods From January 2017 to March 2022,80 patients with lower limb fracture who were admitted to Hankou Hospital of the Central Warzone General Hospital were selected as subjects,and randomly divided into the control group and the observation group,with 40 patients in each group.The control group was treated with interlocking intramedullary nail,and the observation group was treated with early dynamic interlocking intramedullary nail.The fracture healing,excellent and good rate of limb function,bone mineral density,bone regeneration factor and complication rate were statistically compared between the two groups.Results The time of getting out of bed,hospitalization,clinical healing and bone healing in the observation group were shorter than those in the control group(P<0.05).The excellent and good rate of limb function in the observation group was 92.50%(37/40),which was 72.50%(29/40)higher than that in the control group(P<0.05).The levels of bone mineral density(BMD),bone morphogenetic protein 6(BMP6)and fibroblast growth factor 2(FGF-2)in the patients were higher than those in the control group at 3 months after operation(P<0.05).The incidence of complications in the observation group was 7.50%(3/40),which was lower than that in the control group 25.00%(10/40),(P<0.05).Conclusions The early dynamic interlocking intramedullary nail has a significant effect on the treatment of lower limb fracture patients,which can promote the healing of the fracture,shorten the postoperative rehabilitation time,improve the mobility of the affected limb,improve the bone density and bone regeneration factor,and reduce the incidence of complications.It can be recommended clinically.
