血液灌流机串联血液透析机治疗尿毒症皮肤瘙痒患者的护理模式及效果

Model of Care and Effectiveness of Hemoperfusion Machine in Tandem with Hemodialysis Machine in the Treatment of Pruritus Skin Patients with Uremia

目的:探讨尿毒症合并皮肤瘙痒患者采用血液透析机串联血液灌流机治疗期间的护理模式及效果。方法:选取2018年12月—2020年12月九江市第一人民医院收治的75例尿毒症血液透析患者作为研究对象,将2018年12月—2019年12月的37例患者设为对照组,将2020年1月—2020年12月的38例患者设为观察组,对照组患者予以常规护理,观察组患者开展全面护理,对比两组患者治疗前后分子指标、治疗总有效率及皮肤瘙痒程度。结果:治疗后,观察组患者血钙水平较对照组患者低,差异有统计学意义(t=2.32,P<0.05)。治疗后,观察组患者甲状旁腺激素水平和皮肤瘙痒程度较对照组患者低,差异有统计学意义(t=6.38、11.25,P<0.05)。治疗后,观察组患者的抑郁自评量表(self-rating depression scale,SDS)评分、焦虑自评量表(self-rating anxiety scale,SAS)评分均低于对照组患者,差异有统计学意义(t=11.82、6.93,P<0.05)。观察组患者治疗总有效率高于对照组患者,差异有统计学意义(χ^(2)=5.44,P<0.05)。结论:血液灌流机串联血液透析机治疗尿毒症皮肤瘙痒患者,期间开展全面护理,可有效提高治疗效果,减轻瘙痒症状。

Objective:To investigate the mode of care and outcomes during treatment with a hemodialysis machine tandem with a hemoperfusion machine in patients with uremic syndrome combined with pruritus.Methods:75 uremic hemodialysis patients admitted to the hospital from December 2018 to December 2020 were selected for the study,37 patients from December2018 to December 2019 were set as the control group,and 38 patients from January 2020 to December 2020 were set as the observation group.The control group was given conventional care and the observation group carried out comprehensive care.The molecular indexes,the total efficiency of treatment and the degree of skin itching were compared between the two groups before and after treatment.Results:After treatment,the blood calcium level in the observation group was lower than that in the control group,and the difference was statistically significant(t=2.32,P<0.05).After treatment,parathyroid hormone levels and skin pruritus were lower in the observation group than in the control group,with statistically significant differences(t=6.38,11.25,P<0.05).After treatment,the scores of self-rating depression scale(SDS) and self-rating anxiety scale(SAS) in the observation group were lower than those in the control group,and the differences were statistically significant(t=11.82,6.93,P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=5.44,P<0.05).Conclusion:Hemoperfusion machine in tandem with hemodialysis machine to treat patients with pruritus skin in uremia,during which comprehensive care is carried out,can effectively improve the treatment effect and reduce pruritus symptoms.
