隔姜脐灸联合痛泻要方加减治疗腹泻型肠易激综合征疗效观察

Clinical Observation on Umbilical Moxibustion on Ginger Combined with Modified Tongxie Yaofang in the Treatment of Irritable Bowel Syndrome

目的:观察隔姜脐灸联合痛泻要方加减治疗腹泻型肠易激综合征(irritable bowel syndrome,IBS)的临床疗效。方法:将腹泻型IBS患者96例采用随机数字表法分为观察组、对照组各48例。对照组行西医常规治疗,观察组在此基础上加隔姜脐灸联合痛泻要方加减治疗。比较两组患者临床疗效、证候评分、炎症因子水平、神经递质及不良反应。结果:总有效率观察组为93.75%,高于对照组的79.17%(P<0.05)。治疗后两组患者主症与次症评分均下降,观察组下降更明显(P<0.05)。治疗后两组患者肿瘤坏死因子α、干扰素γ及人血清P物质(SP)、5羟色胺、人血浆神经肽Y水平均下降,观察组下降更明显;治疗后两组患者白细胞介素10水平均升高,观察组升高更明显(P<0.05)。对照组不良反应为3例(6.25%),与观察组的4例(8.33%)比较差异无统计学意义(P>0.05)。结论:西医常规治疗基础上加隔姜脐灸联合痛泻要方加减在提高腹泻型IBS患者临床疗效,降低证候评分,缓解炎症反应并降低脑肠肽水平方面优于单独西医常规治疗,且具有较高的安全性。

Objective:To observe clinical effects of umbilical moxibustion on ginger combined with modified Tongxie Yaofang in the treatment of irritable bowel syndrome(IBS).Methods:All 96 IBS patients were allocated to the observation group and the control group by using random number table method,48 cases in each group.The control group performed conventional Western medicine therapy,and the observation group was treated by umbilical moxibustion on ginger and modified Tongxie Yaofang on the foundation of conventional Western medicine therapy.To compare clinical effects,syndrome scales,the levels of inflammatory factor,neurotransmitter and adverse reactions between both groups.Results:Total effective rate of the observation group was 93.75%,higher than 79.17%of the control group(P<0.05).The scores of the main symptom and the secondary symptom reduced in both groups after the treatment,the decrease of the observation group was more noticeable(P<0.05).After the treatment,the levels of TNF-α,IFN-γ,SP,5-HT and NPY decreased in both groups,the decrease of the observation group was more remarkable;after the treatment,the levels of IL-10 increased in both groups,and the increase of the observation group was more significant(P<0.05).The difference had no statistical meaning when three cases suffering adverse reaction(6.25%)in the control group were compared with four cases(8.33%)of the observation group(P>0.05).Conclusion:On the foundation of routine therapy,umbilical moxibustion on ginger and modified Tongxie Yaofang is superior to single routine therapy in raising clinical effects of IBS patients,decreasing syndrome scales,relieving the inflammatory response and reducing the levels of braingut petide,and the treatment has higher safety.
